1. Do the residents appear neat, clean and odor free? Yes
2. Did residents say they receive assistance with personal care
activities? Ex. brushing their teeth, combing their hair, inserting
dentures or cleaning their eyeglasses? Yes
During the visit, residents were dressed and
ready for the day. Breakfast was over and
residents were napping, watching TV, or resting.
3. Did you see or hear residents being encouraged to participate in
their care by staff members? Yes
4. Were residents interacting with staff, other residents & visitors?
Yes
During our visit, the staff interacted with the
residents as needed. This was true in both the
assisted living and memory care sections of the
facility
5. Did staff respond to or interact with residents who had difficulty
communicating or making their needs known verbally? Yes

8. Did residents describe their living environment as homelike? Yes We met with a resident who had recently
transferred from another facility in Chapel Hill,
and was to be discharged from this facility in a
few weeks, as he was there for rehab. He
described the difference between the 2 facilities
as ‘night and day’. He also wondered how the
other facility could pass state inspections. He
was glowing in his praise for the Brookdale
facility and their staff.
9. Did you notice unpleasant odors in commonly used areas? No See note below about odors and carpet
replacement in the entire facility. The residents
were complimentary about the living
environment. They praised the staff, the food,
and the activities.
10. Did you see items that could cause harm or be hazardous? No
11. Did residents feel their living areas were too noisy? No This facility is consistently quiet during our
quarterly visits. No one raises their voice or has
a TV or audio device turned to a high volume,
nor are the staff raising their voice while
conversing with residents..
12. Does the facility accommodate smokers?

13. Were residents able to reach their call bells with ease? Yes
14. Did staff answer call bells in a timely & courteous manner?

15. Were residents asked their preferences or opinions about the
activities planned for them at the facility?
Yes The Activity calendar displayed in the hallway
appeared to be well planned. Evidence of
resident activities such as crafts, Bingo and
other group activity were observed. The facility
halls displayed residents’ artistic talents. We
visited the Memory Care unit, which was a
restricted access unit. During our prior visits,
there was a dressed up Lady mannequin, which
was missing during this visit but we were told
that the mannequin was undergoing a refresh.
There were a variety of setups for games,
activities and colorful decorations, as we were
there on St Patrick’s day.
16. Do residents have the opportunity to purchase personal items of
their choice using their monthly needs funds?
Can residents access their monthly needs funds at their
convenience?

17. Are residents asked their preferences about meal/snack choices?
Are they given a choice about where they prefer to dine?
Yes
Yes
Assisted Living residents ate in the dining
rooms, unless they were ill or wanted to eat in
their room. Memory Care meals were provided
in the dining area in their unit. The new Chef
received much praise from the residents we
spoke with.
18. Do residents have privacy in making and receiving phone calls? Yes
19. Is there evidence of community involvement from other civic,
volunteer or religious groups?
We were informed of a collaboration between
UNC students and the Memory care unit where
some students visited with residents and
engaged in activities.

Are there resident issues or topics that need follow-up or review at a later
time or during the next visit?
Our previous visit in Dec’25 described an odor
issue in some of the hallways. The management
replaced all carpets and so there were no
apparent odor issues that we found during our
visit.
No other issues were mentioned.
