N The facility is clean, free of odors, and uncluttered. Renovation work is complete. Upgrades include fresh paint throughout the inside of the building and improved hallway lighting. Bathrooms have been modified to enhance accessibility and safety, installation of better lighting, new flooring, toilet safety rails and shower grab bars to support safe showering and toileting. 3. Y During the visit, residents reported Follow-up is recommended to review the status of resident that previously scheduled activities activity programming. were not happening like before. The display board posted in the common area was observed to be empty. When asked about current and upcoming activity programming, staff stated that the information was not available to them at the time of the visit. Additionally, the medication room door was observed to be unlocked. Staff indicated they were monitoring the door and it was subsequently secured.
