1. Do the residents appear neat,clean and odor free? Yes
2. Did residents say they receive assistance with personal care Many of the residents boarded the shuttle to be
activities?Ex. brushing their teeth, combing their hair, inserting transported to Charles House for a day of
dentures or cleaning their eyeglasses? Yes activities, only several remained. Residents
seemed pleased to be going to spend the day at
the day facility.

living environment at the facility. Residents were
dressed neatly and appeared to be comfortable
as they relaxed in the lounge area.
Staff checked on residents during the time of our
visit. The TV was on and the staff member
noted it was time to change the channel as a
favorite show was coming on.
