appropriately dressed.

2. Did you notice unpleasant odors in commonly used areas? N The facility is clean, free of odors, and
uncluttered. Renovation work is in
progress. Upgrades include fresh paint
throughout the inside of the building and
improved hallway lighting. Bathrooms have
been modified to enhance accessibility and
safety. Changes include installation of
better lighting, removal of former bathtubs
to increase space, new flooring, and
addition of toilet safety rails and shower
grab bars to support safe showering and
toileting.

actively working on an "ugly sweater elf'
paper craft project. Completed artwork and
crafts from prior activity sessions was
displayed on residents' doors and in the
common area.

visit, the volunteer was observed assisting
residents with creating individualized "ugly
sweater elf' crafts. Completed artwork was
displayed on residents' doors and in the
common area.
One resident reported participating in
Christmas caroling with volunteers from
Alpha Kappa Alpha Sorority. Another
resident shared that members of her
church visited and sang carols with her
and other residents.
Staff reported that some of the craft and
activity materials used by residents were
donated by community partners.

and actively engaged with residents,
demonstrating familiarity with their needs
and involvement in the activities.
