PURPOSE: To approve changes to the Personal Health Services fee schedule as recommended
by the Board of Health and amend the FY 2025-26 County Fee Schedule.
BACKGROUND: Personal Health Services within the Orange County Health Department has
requested two (2) new fees — not previously listed in the fee schedule — be added for COVID-19
vaccine immunizations (effective December 10, 2025). These fees were $0 in the prior fiscal year
based on American Rescue Plan Act (ARPA) funding covering the costs during and after the
pandemic. That funding has now ceased, and the two fees are recommended to be raised. The
new fees will apply to all patients paying by private insurance, Medicaid, or self-pay on a sliding-
fee scale.
Personal Health
Current COVID-19 Vaccine codes for the Fall 2025 season (Effective December 10, 2025)
• 91321: Moderna, mRNA-LNP, 25 mcg/0.25 mL dosage (ages 6 months-11 years) $147.06
• 91322: Moderna (Spikevax), mRNA-LNP, 50 mcg/0.5 mL dosage (ages 12+ years) $161.65
Health Equity Impact
These updated COVID-19 vaccine fees will allow the Health Department to bill at the Medicaid
reimbursement rate, thus keeping the cost low for self-pay patients on the sliding fee scale.
The Board of Health recommended approval of the new fees at its meeting on November 19,
2025.
FINANCIAL IMPACT: Staff projects estimated additional revenue of$4,826 from the two (2) new
fees for the remainder of FY 2025-26.
ALIGNMENT WITH STRATEGIC PLAN: This item supports:
• GOAL 2: HEALTHY COMMUNITY
OBJECTIVE 2. Expand access to quality, affordable healthcare services. (e.g., Crisis
Diversion facility, Medicaid expansion, crisis response, healthy living campaign)
RECOMMENDATION(S): The Manager recommends that the Board approve the new fees as
recommended by the Board of Health and amend the FY 2025-26 County Fee Schedule.
