Resident •rofile Yes/No/N Comments/Other Observations 1. Do the residents appear neat, clean and odor free? Most of the residents were dressed. YES They appeared neat, clean, and odor free. 2. Did residents say they receive assistance with Several residents stated everything was personal care activities? One resident was observed leaving to smoke without noting her absence in the resident log. 13. Were residents able to reach their call bells with ease? YES We received several requests for service. We passed the request along to the nursing assistants and they responded immediately 14. Did staff answer call bells in a timely& courteous manner? Were residents asked their preferences or opinions YES Activities have become much more about the activities planned for them at the facility? important since the arrival of Key.. Bingo is very popular. The residents have Bingo twice a week and can win Bingo Bucks for purchases at the resident store. Prizes include shampoo, deodorant, and snacks. Donations are accepted. 16. Do residents have the opportunity to purchase personal items of their choice using their monthly YES needsfunds? Can residents access their monthly needs funds at their convenience? 17. Are residents asked their preferences about YES The residents are given a choice of meal/snack choices? several options at mealtime. This is a requirement. Kevin took notes. The survey book was in the reception area, but was blocked by the water cart. Kevin indicated he would address. We mentioned the courtyard had not received any attention. The weeds were plentiful and uninviting. Kevin indicated someone had just been hired to address and would start July 1. When we arrived, the new receptionist told us we were not allowed to visit the facility without an escort. With the help of several staff, we were able to convince the receptionist we could visit the facility freely. The facility appeared clean, the staff was friendly and responsive.
