I was the first pharmacist on staff at the UNC Hospital in Hillsborough, and I have been serving in the outpatient oncology clinic since Spring 2015. In my professional scholarship, I have been an advocate for standardization to promote efficiency and reduce errors- I developed and published a checklist to this end for chemotherapy, and the paper won an award from the Journal of Oncology Pharmacy Practice as one of their best articles of the year. My pharmacy experience and familiarity with Orange County have well-prepared me for this position. I currently work with a vulnerable population of oncology/hematology patients, many who are from the more rural areas of Orange County. I spent five years building cancer treatment pathways, which included collaborative decision-making on a team of health professionals and communicating that information out to a large network of providers. In my current position, I co-chair a policy committee with nurses, pharmacists, and physicians across the UNC-Health Network. I have experience writing agendas and moderating meetings. 5 Page 2 of 2 Brian Crandell Reasons for wanting to serve on this board: My family has lived in Orange County Oust outside the Hillsborough town limits)since 2008. Recently, I have been looking at opportunities to get more involved in service within Orange County. When Ms. Deyo brought to my attention that she would be leaving her position, I realized the Board of Health would be an excellent match for my skills and provide an opportunity to promote beneficial health policies across the County. These last few months have shown us how necessary it is to react quickly to changing health scenarios and of the need to prepare for situations like this. I like the idea of collaborating with the diverse team on the Board of Health to tackle such issues. Contribution to the diversity of viewpoints on this board: As an oncology pharmacist, one thing that I encounter daily is the high cost of medical care and its stress on patients who are already in a difficult situation. The solution to this requires better access to life-extending treatment without unexpected costs as well as evidence-based policy and practice to ensure the responsible use of funds. Public health funds are always limited, and it is important to me to use money in a way that most directly improves the health of residents of Orange County and those that are most vulnerable. I also have a personal interest in access to healthcare for young children. As a foster parent, have encountered several kids whose families were simply unable to access necessary medical and dental care. In many cases, the services would have been available at no financial cost to the families, but the parents/caregivers were still not able to get care. But my experiences outside of direct medical care are equally relevant. Reasons for wanting to serve on this board: Healthcare disparities and lack of access are among this country s greatest moral shortcomings, and through research and action we can actually rectify this. Here in Orange County, as in many parts of the country, some of us enjoy excellent healthcare access and outcomes while others struggle to have basic needs met. This is a divisive time in America and healthcare is a great unifier. Every person, regardless of identity or beliefs, should have access to quality healthcare, particularly preventive care which is not only humane but most efficient. I simply cannot imagine not working toward healthcare equity and justice. As a nursing student, I have gained a solid foundation in community health, and worked with social workers and case managers to connect patients with community based resources. Adult Care Home and Nursing Home Community Advisory Committee Please explain how your background, education and experience is relevant to this board Being a Duke cardiothoracic nurse provides me with a unique set of skills and experiences that make me an excellent candidate for the Adult Care Home and Nursing Home Community Advisory Committee. I have a good understanding of complex medical conditions that affects seniors in our community, this can directly contribute in ensuring that residents in nursing homes are receiving evidence-based care. Also, I have experience with advocating for seniors and/or adults with disability both in healthcare settings, and in the community, and also with communicating with patients, families, and multidisciplinary team. I have received dementia care training, and can effectively conduct assessments like functionality assessments (ADLs & IADLs), depression screening, cognitive screening, medication reconciliation using the BEERs criteria and many other geriatric assessment. I am excited at the prospect of working with an interdisciplinary group in Orange County to help form opioid-related policies and develop harm reduction strategies to make opioid use safer, and facilitate the linkage to treatment services for those who hope to begin their recovery process. While this would be the first advisory board I d be participating on, I feel I am uniquely positioned to contribute to the board as a researcher with degrees in Public Administration and Epidemiology, and experience working in the field of public health (specifically in the area of substance use). Further, I am a harm reduction advocate - my primary intention is to reduce the burden of overdose mortality, and a major piece of that is making substance use safer for those who are in active use and do not intend or are unable to enter the recovery process. In terms of experience, for the last 22 years, I directed the Chronic Pain Program in the Division of General Medicine and Clinical Epidemiology, Department of Medicine, at UNC. In that program, we have worked to manage various chronic pain conditions with a minimum of use of opioids. In the past, I saw on the Advisory Board of the NC Governor s Academy for Prevention Professionals, and the Governor s Institute on Alcohol and Substance Abuse, and from the outset, the NC Controlled Substances Reporting System Advisory Committee in the NC Department of Health and Human Services. Years ago, I was on the Orange County Board of Health, serving as its Chair for four of those years. This is say that I am well aware of the issues associated with the opioid epidemic, and that there are a number of solutions to be considered. It would be an honor to be considered for one of the vacancies as there is a lot to be done, and i would like to contribute here in Orange County. As noted previously, I have directed the Chronic Pain Program in the Department of Medicine at UNC. In it, the emphasis is on minimizing opioid use to manage pain while guarding against or managing opioid use disorder(OUD). I've seen the pain and suffering that many patients have and continue to have, and compassion and caring for them is always the best place to start, especially for health care practitioners. If applicable, is there a fiscal impact (i.e., funding, staff time, other resources) associated with these proposed activities (please list). o Environmental Protection and Climate Action - Promote sustainability and resiliency across the County and collaborate with our partners to activate our community, protect and preserve our natural resources, and reduce greenhouse gas emissions. o The Board's ad hoc committee will continue reviewing the Health Department's groundwater protection program in light of the new state legislation to consider the science around the current well rules, implement equitable policies, and help ensure safe groundwater for all. o Healthy Community - Increase equitable access to care and social safety net programming to promote the physical, social, and mental well-being of our community. o The Board is planning to sponsor a Photovoice project, allowing Orange County residents to submit photographs detailing challenges that they face in accessing healthcare. This will help inform planning for future projects and ensure the needs and values of the community are represented in future work.
