(Required for Nursing Homes Only) Comments/OtherResident Profile Yes/No/ Observations NA 1. Do the residents appear neat, clean and odor free? YES We observed many residents still in bed, still in bedclothes. 2. Did residents say they receive assistance with Several residents stated they received personal care activities? Were residents asked their preferences or opinions YES Activity director had just been replaced about the activities planned for them at the facility? and consequently books. Games, puzzles were in short supply. Activity calendar was sparse. This facility could benefit with more community participation. Several residents mentioned they were bored. 16. Does the facility have a Resident's Council? YES The President indicated things were in Family Council? No transition and seemed very patient. Areas of Concern Yes/No/ Exit Summary NA Are there resident issues or topics that need follow-up or review at a later time or during the next visit? Upcoming will be the 4t" administrator this facility has had this year. The interim administration staff will be leaving shortly. The activity program has had a set back with a transition as well. This program aspect is seriously lacking and residents expressed boredom. They at least need self-directed activity which does include the TV! The residents do not seem to have access to beauticians. This would help the residents if they had access to haircuts, etc. It was mentioned that a lot of residents were still in bed in pajamas with breakfast trays. We arrived at the facility at 10AM. One resident expressed interest in getting to her churches homecoming. We asked that this be shared with the social worker. Miss Kim indicated that transportation was at least $20.00 and she would see if the resident account allowed it. This Document is PUBLIC RECORD.
