in bedclothes.

problem.

manner? YES checked in with them immediately

other civic, volunteer or religious groups? participated in activities. Currently
scheduling has slowed due to the
departure of the activity director. The
office still remains in the basement and
only a few books and puzzles were in
one of the lounge areas.
20. Does the facility have a Resident's Council? YES Again we spoke with the President of
Family Council? No the Resident's Council. He indicated the
facility was undergoing a change and it
would be sorted out.

Are there resident issues or topics that need follow-up or With the departure of the administrator,
review at a later time or during the next visit? the facility has undergone some
changes. The activity director also left
along with the front desk receptionist.
• The survey book was up to date,
but needs to be displayed in a
more visible area.
• One resident was unhappy with
the hardened bread she was
served. The administrator
indicated that staff could ensure
softer toast, but the resident
needed to express this to them.
• The administrator indicated the
residents have gotten use to
being served their meals in their
room because of the fire and
construction that followed. She
asked for methods to get the
residents to the dining room and
several suggestions were made.
• Some of the residents were
observed to be in bed still in
their bedclothes while staff
chatted in small groups. The
administrator indicated she
would address this with
supervisors and staff.
• The courtyard was addressed.
The plantings were dead and the
lawn furniture was broken and
presented a safety risk.
• Census is low. 140 bed facility
provides care for 83 people
currently.
