PURPOSE: To approve budget ordinance and grant project ordinance amendments for fiscal
year2003-04.
BACKGROUND:
Department of Social Services
The Department of Social Services has received notification from the State of additional
funds for the current fiscal year for the following programs:
Foster Care - An additional receipt of State reimbursement funds totaling $20,000
due to the number of children that are currently in foster care. The department is
currently serving 20 more children than last fiscal year. These additional funds bring
the current budget far this program to $61,723 for FY 2003-04. This budget
amendment provides for the receipt of these funds. (See Attachment 1, column #1)
Home and Community Care Block Grant -this State allocation of federal funds
totaling $34,599 will be used to enable low-income, minority individuals to defer or
prevent institutionalization by providing quality services to meet basic needs. These
funds bring the current budget to $34,599 for FY 2003-04, as the department did not
budget for these funds in the FY 2003-04 budget. This budget amendment provides
for the receipt of these funds. (See Attachment 1, column #1)

c, Long Term Care Screening (EDS) -this State allocation of federal funds totaling
$10,000 will be used to enable low-income, minority individuals to defer or prevent
institutionalization by providing quality services to meet basic needs. These
additional funds bring the current budget to $35,000 for FY 2003-04, This budget
amendment provides for the receipt of these funds. (See Attachment 1, column #1)
d. IV-B Adoption Vendor Payments -these additional State funds totaling $5,000 are
payments for allowable costs on behalf of children who are eligible for adoption
assistance. These additional funds bring the current budget to $20,000 for FY 2003-
04. This budget amendment provides for the receipt of these funds. (See
Attachment 1, column #1)
Department on Aging
2, The Department on Aging has received additional revenues in FY 2003-04 far Adult Day
Care operations related to private pay revenues totaling $12,000, as well as reimbursement
funds from the Home and Community Care Block Grant program totaling $9,052. These
funds will be used for personnel costs related to the Adult Day Care operations. This budget
amendment provides far the receipt of these funds, (See Attachment 1, column #2)
Health Department
3, During the budget review process for fiscal year 2003-2004, the Board of County
Commissioners expressed a desire to continue several of the Health Department's grant
supported programs that lost funding or were experiencing reductions in funding. These
programs and their associated positions were extended with the balance of their revenue
shortfall to come from the Human Services Social Safety Net. The following are these grant
supported programs:
Enhanced Child Service Coordination
Provides a central portal of entry to services for children with developmental delays as well
as ensures that appropriate referrals to agencies throughout the County are made. Case
Management is also provided for the more complex cases. It receives Smart Start funding in
the amount of $46,725.
Community Health Awareness Project (CHAP)
Provides the only outreach program to the Hispanic community and other hard-to-reach
populations by training lay health advisors who provide information to neighbors and
acquaintances (36 lay health advisors have been trained in the three years of this program).
CHAP has produced two photo novellas on topics identified by the lay health advisors (Child
Safety in the Kitchen and Childhood Poisoning Prevention Tips) and has received the
Outstanding Health Education Project Award, the Gold Project Award, and was a featured
finalist far the Glaxo Smith Cline Child Health Recognition Award, Smart Start did not
recommend continued funding for this program for FY 03-04.
Healthy Carolinians
Coordinates health planning and implementation for identified high priority community health
issues such as child abuse prevention, teen pregnancy prevention or the prevention of
chronic diseases. This program also conducted the quadrennial community assessment for
the Health Department, and produces an annual report and community forum reporting on
progress made in addressing issues identified in the community priorities, The State has
provided partial funding for this program for two years; this funding was eliminated in FY 03-
04, The Health Department has secured funding through a Kate B. Reynolds grant far three
years of diminishing funding, as well as acquired funding in the amount of $5,000 from UNC
Hospitals..
Intensive Home Visiting
Serves families that are at high risk far child abuse and neglect through counseling,
education, and coordination of services and are provided in the home with families on a
weekly basis, State funding in the amount of $40,745 was eliminated in FY 03-04; Smart
Start funding has been increased by $14,000 to assist with the State shortfall,

This amendment would transfer $87,260 from the Human Services Social Safety Net for the
programs/services specified above, With this transfer, $150,952 remains in the Human
Services Social Safety Net far FY 2003-04. (See Attachment 2, Smart Start Grant
Project Ordinance, Aftachment 3, Healthy Carolinians Grant Project Ordinance, and
Aftachment 4, Intensive Home Visiting Program Grant Project Ordinance)
FINANCIAL IMPACT: Financial impacts are included in the background information above.
RECOMMENDATION (S): The Manager recommends that the Board of County Commissioners
approve the attached budget ordinance and grant project ordinance amendments.

Smart Start Program
Grant Project Ordinance
Be it ordained by the Orange County Board of County Commissioners that pursuant to Section 13,2
of Chapter 159 of the General Statutes of North Carolina, the following grant project is hereby
adopted.
Section 1. The project authorized is the Smart Start Program as awarded to the Orange County
Departments of Social Services, Health, Library and Cooperative Extension by the State of
North Carolina and the Orange County Partnership far Young Children (OCPYC).
Section 2. The officers of the County are hereby directed to proceed with the grant project within
the grant document, and the rules and regulations of the State of North Carolina.
Section 3. The following revenue is anticipated to be available to complete this project:

sufficient specific detailed accounting records to provide the accounting to the grantor
agency required by the grant agreement and federal and state regulations,

Section 9. Positions authorized through this grant project include:

Subsidy program as authorized by the Orange County Board of County
Commissioners on January 18, 1994.
1,0 full time equivalent position in the Health Department Enhanced Child
Service Coordination Project as authorized by the Orange County Board of
County Commissioners on December 5, 1994,
Funding for these positions is contingent upon the General Assembly's allocation of
Smart Start funds.
1.0 full time equivalent position in the Health Department Community Awareness
Project as authorized by the Orange County Board of County Commissioners on
September 7, 1999.
Section 10. This project period begins January 18, 1994, and is valid through June 30, 2004.
Subsequent funding of this grant project is contingent upon the availability of funds
sufficient to cover expenditures.
Section 11. This ordinance supersedes previous Smart Start Program Grant Project Ordinances.
Adopted this 4th day of May 2004.

Healthy Carolinians Program
Grant Project Ordinance
Be it ordained by the Orange County Board of County Commissioners that pursuant to Section
13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is
hereby adopted.
Section 1.The project authorized is the Healthy Carolinians Program in the Orange County
Health Department. The purpose of this grant program is to provide a plan to guide
and assist the County in planning and implementing health care strategies to promote
healthy lifestyles, improve health status and prevent premature death and injury for all
residents in the County regardless of age, race, income or education.
Section 2.The officers of the County are hereby directed to proceed with the grant project within
the grant document and the rules and regulations of the grantors.
Section 3.The following revenue is anticipated to be available to complete this project:

Intensive Home Visiting Program
Grant Project Ordinance
Be it ordained by the Orange County Board of County Commissioners that pursuant to Section
13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is
hereby adopted.
Section 1. The project authorized is the Intensive Home Visiting Demonstration Program as
awarded to the Orange County Health Department by the North Carolina Division
of Women and Children's Health, Department of Health and Human Services.
The planned project period continues until June 30, 2003. This grant provides
funding for 2.5 permanent, time-limited positions through the grant period. The
grant provides funds to decrease child abuse and neglect, improve parent-child
interaction, enstare adequate use of preventive family support services, improve
children's health and development, and improve the service delivery system in
Orange County by decreasing fragmentation and duplication.
Section 2. Under the terms of the agreement, Orange County will administer the grant as
fiscal agent. The officers of the County are hereby directed to proceed with the
grant project within the rules and regulations contained in the grant document,
the rules and regulations of the North Carolina Department of Health and Human
Services and the budget contained herein.
Section 3. The following revenue is anticipated to be available to complete this project:
