Research has shown that the earlier one starts using tobacco products, the more difficult it is for that individual to stop using those products. Each year, 24,090 children (66 per day) in North Carolina start smoking. The use of tobacco products costs the State $4,75 billion in medical costs and lost productivity annually. The following statistics from a survey conducted in 2001-2002 show that this is an important problem for Orange County as well: • 12% of Orange County Schools' Stn graders that were surveyed reported they had smoked in the last 30 days; Chapel Hill Carrboro City Schools reported 3%, • 24% of 9th graders surveyed reported smoking in the last 30 da~s in Orange County Schools; Chapel Hill Carrboro City Schools reported 20% of 10 graders smoking. • 17% of Orange County Schools' 9th graders report that they had tried to quit within the past year; 10% of 10th graders in Chapel Hill Carrboro had attempted to quit, • 27 middle and high school students were suspended for violating the school board policy on tobacco use in Orange County Schools. Tobacco use is one of the primary preventable causes of cancer and cardiovascular disease (heart disease). About 200 people die from cancer each year in Orange County and another 400 people are diagnosed with cancer each year in this County alone. One hundred and sixty people die from heart disease each year and more than 860 people are hospitalized each year for heart related conditions. Statistics over the last eight years show that the County has made little progress in improving these numbers. 2 This grant was awarded to the Health Department beginning March 1, 2003 by the North Carolina Health and Wellness Trust. It seeks to prevent youth from beginning tobacco use and assists those that are users to stop. The grant is a collaborative effort between the Orange County Schools, the Chapel Hill/Carrboro City Schools, the Orange County Department of Social Services, and the Orange County Health Department. The overall focus of the project is on reducing tobacco and marijuana use among middle and high school students and on developing the skills necessary to prevent the use of these substances. A coordinator was hired and began work in August of 2003. Attached to this abstract is a Summary of Accomplishments in the seven months since the coordinator was hired. A few notable accomplishments include: • 14 peer educators from each school system have been recruited and are being trained • An Alternative to Suspension program was approved on a trial basis in 2003-04 in Orange County Schools high schools and middle schools • Adolescent Parenting Program participants received skill building interventions to reduce exposure to secondhand smoke in the home • Reached 50 private providers with information an initiating tobacco prevention and cessation conversations with youth patients • Youth peer educators were successful in convincing AMF-Durham Bowling Lanes to make half of the bowling lanes smoke free (increase from 10 to 16). The project's funding duration is three years and began on March 1, 2003, the original date of the acceptance of the grant; with each year's funding being renewable upon successful completion of project activities. FINANCIAL IMPACT: The original grant amount for Year Two was $75,506. There was approximately $47,459 in unspent Year 1 grant funds due to the late hiring date for the project coordinator. The project staff is requesting that the Health and Wellness Trust allow a carry forward of $18,967.13 to Year Two with the remainder being carried forward to Year Three. The total amount of the grant over the three year period remains at $232,848. The Year Two allocation will cover the salary and benefits of the Senior Public Health Educator/Project Manager and includes all operational expenses for the project. In-kind contributions by the County include office space and supervisory time. RECOMMENDATION(S): The Manager recommends that the Board approve receipt of Year Two of the grant fund for Youth Tobacco Use Prevention Project and authorize the Board Chair to sign the grant agreement with the North Carolina Health and Wellness Trust Commission. SUMMARY OF ACCOMPLISHMENTS Youth Tobacco-Use Prevention Project, Orange County Health Department The following is a summary of highlighted activities carried out as part of the Orange County Health Department's Youth Tobacco-Use Prevention Project, funded by a grant from the Health and Wellness Trust Fund Commission (HWTFC) of North Carolina. July, 200.3 Orange County Schools Pass 100% Tobacco-Free Schools Policy The Orange Co. Health Department Director consulted with the Orange County School District on their decision to go 100% Tobacco Free for the 2003-2004.. The Health Department provided assistant superintendent with educational materials and links to cessation resources. August, 200.3 Youth Tobacco-Use Prevention Project Manager Position Filled The Orange County Health Department hired a Project Manager for the Youth Tobacco-Use Prevention Project in early August. The project manager, with the Health Promotion and Educational Services (HPES) Division Director, assumed responsibility and management of the project.. Project Manager Training The Project Manager was trained in areas relevant to the effective implementation of the grant's goals and objectives. The Project Manager also met with grant evaluators and state technical assistance experts regarding proper progress reporting and best practices, September-October, 2003 Proiect Partnership Development The Project Manager and Director of the HPES Division Director established community partnerships and collaboration commitment from: • Orange Coanty Schools through the endorsement of the project by Superintendent, Dr. Shirley Carraway, principals of Orange High School, Cedar Ridge High School, C.W.. Stanford Middle School, A.L. Stanback Middle School, the Healthful Living Coordinator, Donna Williams, and high school health and PE. teachers.. • Chapel Hill/Carr°boro City Schools through the endorsement of the project by Superintendent, Dr. Neil Pedersen, Health Coordinator, Stephanie Willis, high school substance abuse coordinators, and Student Health Advisory Council (SHAG). • Adolescent Parenting Program (APP) of Orange County through the endorsement of the project by Program Coordinator, Valerie Russel, and high school case coordinators. Community-based Outreach Project Manager attended Octoberfest fair in Efland-Cheeks, Equity and Excellence Day at Smith Middle School, and Latino Family Day and United Church (Chapel Hill), Youth were asked to sign smoke-free pledges and given tobacco prevention education materials. Alternative to Suspension (ATSI Program Development A draft proposal for the ATS Program was developed and sent to principals at OCS high schools and middle schools, as well as the superintendent, to review. School-based Outreach 4 Project Manager entered classrooms, visited lunch sessions and Friday night football games at both Orange High School and Cedar Ridge High School to inform students of the opportunity to become a peer educator with the project and initiated application process, Assessment of Exposure to Secondhand Smoke Among Adol. Parenting Program Participants Administered a survey to assess the amount and location of secondhand smoke exposure for participants of the Adolescent Parenting Program. Results to be used to develop apopulation-specific intervention to reduce exposure to secondhand smoke. November, 200.3 Development of Year I Survey Assessment of OCS 100% Tobacco-Free Schools Policy Project Manger met with OCS Healthful Living Coordinator and the North Carolina Tobacco Prevention and Control Branch to develop a survey instrument assessing the progress of the 100% Tobacco Free Schools Policy.. Peer Educator Recruitment Fourteen peer educators were recmited from both Orange High School and Cedar Ridge High School to comprise the 2003-2004 Tobacco Reality Unfiltered (TRU) (OCS) youth group. Secondhand Smoke Intervention with Adol. Parenting Program Participants Adolescent Parenting Program Participants received skills building intervention around reducing exposure to secondhand smoke in the home. Participants role-played and practiced skills needed to advocate for smoke-free homes for themselves and their child. December, 2003 School-based Outreach Project Manager entered classrooms and visited lunch sessions at both East Chapel Hill High School and Chapel Hill High School to inform students of the opportunity to become a peer educator with the project and initiated application process. Orange County I-Iealth Care Provider Education Distributed best practices guidelines to Orange County Health Care Provider for initiating tobacco prevention and cessation conversations with youth patients. Reached approximately 50 providers. Attended National Tobacco or Health Conference Project Manager traveled to National Tobacco or Health Conference to further training and expertise development in youth tobacco-use prevention and cessation. Board of Health Accepts N.O.T. (Not on Tobacco) Mini Grant to Initiate Youth Cessation Prod Mini grant in the amount of $1,500 secured to initiate youth tobacco cessation classes in OCS. January, 2004 News of Orange Monthly Column Granted Project Manger negotiated and received monthly column with the News of Orange to cover issues related to youth tobacco-use prevention. Youth involvement in the colurmr will be developed,. Peer Educator Training 2003-2004 TRU Group (OCS) peer educators begin training in youth tobacco-use prevention curriculum. Tobacco Reality Unfiltered Website Linked to All OCS Middle and Higlr School Webpages All high schools and middle schools within the Orange County School district posted and activated a link to the tobaccorealityunfiltered.com website. Alternative to Suspension (ATS) Program Approved by OCS Principals OCS high school and middle school principals agreed to implement the ATS program on a trial basis for the remainder of the 200.3-2004 school year and revisit the program at the end of the year.. Peer Educator Recruitment Fourteen peer educators were recruited from both L^ast Chapel Hill High School and Chapel Hill High School to comprise the 200.3-2004 Tobacco Reality Unfiltered (TRU) (CHCCS) youth group. Tebruary,2004 Peer Educator TraininH 200.3-2004 TRU Group (OCS and CHCCS) peer educators continue (OCS) /begin (CHCCS) paining in youth tobacco-use prevention curriculum. Securing Summer Intern from UNC-CH School of Public Health Summer intern from UNC-CH School of Public I-Iealth recruited to assist in peer educator manual development and facilitation of youth tobacco-use prevention and cessation services, Durham-Orange Counties Youth Empowerment Retreat 35 youth (25 from OC and 10 from DC) attended the retreat and were trained in Tobacco 101, Media Literacy, Policy Advocacy, Smoke-Free Env., Action Planning, Communicating your Message, and group identity development. Youth peer educators were successful in convincing AMF-Durham Lanes bowling alley to make half of the bowling lanes smoke free (increase from 10 to 16). Received regional TV News coverage on WB22 and NBC Ch. 17 March, 2004 Peer Educator Trainine 2003-2004 TRU Group (OCS and CHCCS) peer educators continue training in youth tobacco-use prevention curriculum. Attendance at State-wide Youth Tobacco-Use Prevention Leadership Institute 6 peer educators selected to attend state-wide training event and represent Orange County's TRU Group. Students then led trainings with fellow TRU Group members on subjects learned during the institute.. School-based Outreach TRU Group (OCS and CHCCS) developed a Tobacco 101 presentation to be implemented by peer educators in OCS and CHCCS middle schools. Members from Cedar Ridge High School used the presentation and other activities during the health fair at that school. Peer L^ducators Lead March 31s' "Kick Butts Dav" Activities in OCS and CHCCS Schools TRU Group peer educators led tobacco prevention education activities in Orange, Cedar Ridge, East Chapel Hill, and Chapel Hill High Schools as part of the national Kick Butts Day.. Grantees are required to attend all regional and Statewide Tobacco Prevention and Control Coalition meetings including: New Grantee Kickoff event, Local Coalition C,oordinatormeetings, Youth Summits, State TPC Training Event. A minimum of $1,500 per individual (up to there) attending these events should be included for travel and lodging to support these expenses. In addition, grantees may want to budget an additional $1,800 per individual for travel, lodging and per diem (up to two) to attend the annual Tobacco Use and Prevention Training Institute (TUPTI) and another $1,200 perindividual (up to two) to attend aself-selected tobacco control national or regional conference/workshop. N,G Health and Wellness Trust Fund Comnilssion Teen Tobacco Use Grant (Unamended) Renewal Year 2 GRANT AGREEMENT BETWEEN THE NORTH CAROLINA HEALTH AND WELLNESS TRUST FUND COMMISSION AND ORANGE COUNTY HEALTH DEPARTMENT AGREEMENT TO RENEW FOR A SECOND CONSECUTIVE YEAR MARCH 1, 2004 THROUGH FEBRUARY 28, 2005 AGREEMENT TO AMEND SECTION H, SECTION IV, SECTION VII, SECTION VIII AND SECTION XVI OF THE AGREEMENT In accordance with the Agreement between them, the North Carolina Health and Wellness Trust Fund Commission (hereinafter referred to as the "Commission") and Orange County Health Department ("OCHD"), a government entity (together "the Parties") hereby agree to renew and amend the Agreement between them effective March 1, 2003. This renewal is for the second year of a program to provide acommunity-based tobacco use prevention and cessation program for teenage North Carolina residents as is more fully described in the Grant Agreement between the Pazties dated March 1, 2003 ("the Project"). As part of this renewal, the Commission will provide OCHD no more than $75,506, dispersed in 12 equal monthly payments, for the Project subject to the availability of funds and satisfactory performance by OCHD of the duties assigned to it by the Grant Agreement and this renewal and anlendments, Not withstanding Subsection B of the Section V of the Grant Agreement, an additional $ 18,967.1.3 which represents funds allocated by the Commission to the first year of the Project but not spent by GCHD in the first year of the Project shall be included in the budget for Year 2 of the Project subject to the Conunission's prior approval of the specific budget line items and accompanying narrative of how these fiords will be spent. Any remaining unspent grant money from the first year of the Project shall be carried forward to the final term of the Grant Agreement, such carry-over to be used to extend the length of that final term should the Grant Agreement be renewed for a final term.. The budget for Year 2 of the Project is attached and is hereby incorporated by reference into this Renewal Agreement. The Parties agree that this budget for Year 2 shall be the budget to which they shall both adhere for the second year of the Project. Additionally for good and valuable consideration the parties agree that the original agreement shall be amended to include the following underlined new subsection F in Section II. Scope of the Grant: F. Each community/school grantee must develop an annual action flan based on the program of work in their approved rant application that is aliened with their approved annual budget. The annual action plan must be developed under the guidance of the Tobacco Prevention and Control Branch and approved by the Health and Wellness Trust Fund Commission. Any changes in the action plan must be approved by the Health and Wellness Trust Fund Commission in consultation with the Tobacco Prevention and Control Branch. Additionally for good and valuable consideration the parties agree Section IV Funding, subsection C, of the original Agreement shall be amended to reflect a total award of $5000 for capital expenditures or equipment expenses. And the Parties agree that for good and valuable consideration Section VII. Reports of the original Agreement shall be amended by adding the following underlined language in a new subsection G: G The Grantee shall also provide annual~orts pursuant to N.C. Gen. Stat. 143-6.1. Pursuant to that statute if the Grantee is a non-state entity and receives $300,000 or more of combined state funds annually, if agrees that it shall file an audited financial statement with the Commission and the State Auditor in a format and at a time as prescribed by the State Auditor along with any other reports the State Auditor may require. Also pursuant to that statute if the Grantee is a non-state entity and receives more than $15,000 but less than $300.000 of combined state funds annually, it agrees that it shall file annual accounting with the Commission in a format prescribed by the State Auditor. The 10 Grantee further agrees that if it does not file reports required by the Auditor pursuant to G.S. 143-6.1, the Commission cannot disburse grant funds to it. For good and valuable consideration the Parties further agree that Section VIII. Project Records is hereby amended at Subsections B and E. to include the following underlined language and to exclude the language that is struck through: Section VIII. Proiect Records. B.. The Grantee shall retain all financial records, supporting document and all other pertinent records related to the Project for a period of three-(~j five 5 years from the Termination Date. In the event such records are audited, all Project records shall be retained beyond such t#~ee-year five-vear period until any and all audit findings have been resolved. 501 In the event the Grantee dissolves or otherwise goes out of existence before the Termination Date or before fihree five years from the Termination Date has elapsed, records produced under this contract will be fumed over to the Commission.. Finally, the Pazties agree that Section XVLNotice of the original Agreement shall be amended to provide for a change in the Commission's address by deleting the language below that is shuck through and by adding the language below that is underlined: If to the Commission: Mr'.
