PURPOSE: To consider approving an agreement with UNC Family Practice for after hours
medical coverage at Orange County Jail.
BACKGROUND: The Health Department began providing jail health services on July 1, 2001
for the Orange County Jail through an agreement with the Sheriffs Office. Primary jail health
services are provided by an on-site family nurse prect8ioner employed full-time by the health
Department for the jail health program. In order to cover the needs of inmates after hours when
the nurse practitioner was not available,Me Department had received approval to hire a pan-
time registered nurse to cover evenings and weekends, After four attempts to mount apan-
time nurse,Me health department bought an alternative strategy to cover evenings and
weekends. An agreement was developed with Me UNC Family Practice faculty for evening and
weekend consultation on medical needs of incoming or established inmates. The on-call faculty
provide medical and pharmaceutical consultation and guidance to jail staff between Me hours of
4:30 pm and 7:30 am. Monday through Firms, weekends, and holidays. The annual cost for
this agreement is$6.000. The beat of the part-time nurse was estimated to be$29,000.
Therefore,the cost savings in using this alternative is substantial.
FINANCIAL IMPACT: The$,000 in this agreement is considerably less than the$29,000
estimate for this service in the 2001-2002 budget. The$6,000 is included in the Sheriffs
budget for jail health services and will be utilized he pay this agreement.
RECOMMENDATION(S): The Manager recommends that Me Board approve the agreement
and authorze the Chair to sign, pending Me approval of the 2002-2003 budget.

UNC Family Practice
And
Orange County Sheriffs Office in collaboration with
Orange County Health Department
Whereas the Orange County Health Department and the Orange County Sheriffs
Office are both departments within the County of Orange; and
Whereas the Orange County Health Department provides Jail Health services for
the Orange County Sheriffs Office;
And whereae the Health Department has read for the provision of after-hours
medical consultation fir the jail;
And whereas UNC Family Practice is willing to provide such sandoes;
Therefore the parties agree to the following:
UNC Family Practice shall:
Provide medical and pharmaceutical consultation and guidance to jail
staff by telephone as requested between the hours of 4:30 PM and x:30
AM Monday thin Friday, weekends(4:30 PM Friday through 7:30 AM
Monday), and holidays.
• Document consultation or guidance provided and make available via fax
or a-mail the next day to Jail Health staff, as requested,to assure
continuity of care.
• I nvoice quarterly the Orange County Sheriffs Office for service rentleretl
in accordance with this agreement.
Orange County Sheriffs Office in collaboration with the Orange County Health
Department shall:
• Reimburse UNC Family Practice at the rate of$500 per month ($1500 per
quarter)for the above after-hours medical consultation.
• Review invoice and make payment for service rendered in accordance
with this agreement.
This agreement shall he In effect tram July 1,2002 through June 30, 2003.
UNC Family Practice agrees to maintain professional liability Insurance and
responsibility for provision of all consultation and guidance provitled by on-call
staff.They further agree that they are operating as independent contractors and
are not eligible for any benefits available to employees, Including workers
compensation coverage.

UNC Family Practice agrees to hold Orange County Government,the Orange
County Health Department, and the Orange County Sheriffs Once harmless
from any and all liability as a result of implementing the elements of this
agreement.
Any parry may terminate this agreement with 30 days written notice to the other
parties.
In witness whereof, UNC Family Practice, Orange County Heats Department,
and Orange County Sheriffs Office have executed this agreement in triplicate
originals, one of which is retained by each of the parties.
