PURPOSE: To approve the Home and Community Care Block Giant Funding Plan.
BACKGROUND: The State Department of Human Resources established the Home and
Community Care Black Grant in July 1, 1992. The grant did not create new funding streams.
but combined funds appropriated to the Department on Aging, Department of Social Services,
and Joint Orange Chatham Community Action Agency, (JOCCA) to administer programs for
senior citizens.
The block grant requires the Board of Commissioners to approve the recommended funding
plan. The recommended funding for the 2002-2003 year Is$399,001. The amount is $5,811
lass than 2001-2002 grant amount of$404,012. The Home and Community Care Technical
Advisory Committee reviewetl the plan on April 23, 2002 and supported the continual of current
funding levels. The Advisory Board on Aging also reviewed the committee's plan on
June 14, 2002 and recommended the approval. The Funding Plan is attached.
FINANCIAL IMPACT: The Home and Community Care Block Grant to the County will decrease
$5,811 for fiscal year 2002-2003.
RECOMMENDATION(S: The Manager recommends that the Board approve me 2002-2003
Home and Community Care Block Grant Funding Plan and authorize the Chair to sign grant
documents.

Congregate Nutrition
JOCCA has six Congregate Meal Sites located in

Orange County. A site manager is located at each
site and is responsible for the tlsily operation of
nutrition programs and meals. There are also
three coordinators of services responsible for the
coordination, outreach, referral and provision of
services to low-income families antl intlivitluals.
These services inclutle, but ere not limitetl to
weetherization, housing rehabilitation assistance,
financial emergency assistance, Older Workers
Pro ram, and information and referral.

1. Services shall be provided in accordance with requirements set botch a :
a) The County Funding Plan;
b) The Division of Aging Home and Community Care Block Grant Procedures
Maand for Community Service Providers; and
c) The Division of Aging Services Standards Manual, Volumes I through IV.
Community service providers shad monitor any contracts with providers of Block
Grant services and tale appropriate measures to ensure that services are provided in
accordance with the aforementioned documents.
2. Priority .shin he given to providing services or these older persons with he greatest
accurate or social made, with pacticalar attention in low-income minority
ladividuals. The service needs of low-mcame minority elderly will be addressed in

3. The following service authorization activities will be carried out Or conjunction with

a) Eligibility determination;
b) Client intake/registration;
i) Client assesemmUreasseeements and generly visits, as appropri ce;
d) DetermWng The amount of services to be received by the client; and
e) Reviewing cost sharing/volummy contributions policies with eligible clients.
4. All licenses,permits, bow%, and insurance necessary for carrying out Block Grant

5. As specified in 45 CFR 92.36(b)(11), community service providers shop have
procedures for senling all connection and administrative issues raising out of
pro tmemem of services through the Block Gmur- Community service providers
shall have procedures governing the evaluation of bids for services and procedures
through which bidders and contracted providers may appeal or dispute a decision
made by the community service provider.
6. Applicant/Client appeals shall be addressed as specified in Scction 7 of the Division
of Aging Home and Community Cure Black Craft Manual for Community Service
Providers, dared February 17, 19W.
7. Congruency service providers the responsible for providing or arranging for the
provision of required land match, re specified on the Provider Sevices Surmnary,
(DOA 732). Local match shall be expended simultaneously with Block Grant
funding.
g. Providers expending 8300,000 or name in federal financial assistaace through the
Home mad Community Care Black Grain in combination with other federal
funding shall receive an annual wdepeadent audit which meets the requirements of
the Division of Aging Program Audit Guide for Aging Services, applicable North
CaroWO General Stamens and I.ncal Goverrmrem Commission requirements, and
ObfB Circular A-133. for-profit community service providers shall have an annual
compliance audit which meets the regnlmemems of A-133- The audit shall be
p reformed within nine (9)momhs of the close of the provider a fiscal year. Upon
completion of the audit, non-profit and for profit providers shelf provide a copy of
the audit report and any opinion letter simultaneously to the County and the Area
Agency Federal fund;will cot reimburse the cost of a single audit if the cool of all
federal Ponds expended by the provider is less than$300,000

Requirements Regarding Client Rights

Agencies Providing In-Home Services through the
Home and Community Care Block Grant for Older Adults
As a provider of one or more of to services listed below, our agency agrees to notify all
Home and Community Care Block Grant clients receiving any of the below listed serAces
provided by this agency of the their rights as a service recipient. Services Included In this
assurance include:
-In-Home Ade
-Home Care(home health)
-Housing and Home Improvement
-Adult Day Care or Adult Day Health Care
Notification will Include, at a minimum, an oral review of the information outlined below as well
as providing each service recipient with a copy of the information in written turn. In addition
providers of in-home services will establish a procedure to document that client rights
information has been discussed with in home services clients (e.g. copy of signed Client Bill
of Rights statement).
Client Rights Information to be communicated to service recipients will include, at a minimum,
the right to

service to be provided and any change in services)
that may affect the well-being of the part iapate;
-participate In planning and chaoging any in-home service
provided unless the client is adjudicated incompetent;

through which bidders and contractual providers may 4ppeal or dispute a decision
made by the community service provider.

of Aging Home and Community Cue Block Cram Manual for Community service
Providers, dated February 17, 1997.

As a provider of one or more of the services listed below, our agency agrees b nobly all Home
and Community Care Block Grant diems receiving any of Me below listed services provided by
this agency of their rights as a service recipient Services in this assurance include:

Notification will include, at a minimum, an oral review of the information outlined below as wall
as providing each service recipient with a copy of the information in written form. In addition,
providers of in-home services will establish a procedure to document that diem rights
informaton has been discussed with inhome services clients(ep. copy of signed Client Bill of
Rights statement).
Clients Rights information to be communicated to service recipients will include, at a minimum,
