PURPOSE: To consider renewing an agreement between Orange County and the UNC
Department of Emergency Medidne concerning medical direction of Orange County's
Emergency Medical Services (EMS)program.
BACKGROUND: Orange County provides Advanced Life Support(ALS) EMS service. Under
State EMS regulations, a County that provides ALS service must do sa under the guidance and
medical direction of a physician. The County receives medical direction through the University
of North Carolina's Department of Emergency Medicine from Dr.Jane Brice and other
emergency medicine physicians.
The Medical Direci responsibilities involve the medical management of all aspects of the
EMS system. One area of work Is the emergency medical dispatch program in the 9-1-1 center,
where the Medical Director has responsibility for providing protocol development. medical
training, protocol compliance monitoring, quality assurance, and approves telecommunirators
for EMD certification. Another area ofwork is the first responder defibrillation program, in which
the Medical Director establishes the level of practice for first responder personnel, defines the
training requirements for them to operate automatic defibrillators,and ensures annual training
and testing for the defibdllaticn program. The largest area of work for the Medical Director is
the medical management of the Initial Response Paramedic program. The Medical Director
determines the medical standard of care and develops appropriate treatment protocols for use
by the paramedics. The Medical Director oversees the initial and ongoing training programs,
and is responsible for providing training, monitoring skill proFlUancy and conducting regular
performance evaluations for the Initial Response Paramedics.
The agreement went the Hospital also ebpuletes that the Medical Director have an appropriate
number of assistant medical directors to assure availability at all times. The Medical Director
serves as a consultant forthe Paramedics in the field on patient rare issues. The Medical
Director meets weekly with EMS staff to pi consultation and review of medical Issues.

In addition to physician Medical Direction services, ZINC Hospitals also provides a nurse liaison
who is responsible for maintaining positive interactions between EMS personnel and nursing
staff in the emergency department. The nurse liaison also facilitates skill evaluations, special
training eeesions and technician reviews with the staff training coordinator, and assists in writing
the required two-year paramedic training program for state approval. The Hospital provides
limited computer support for downloading Information from medical equipment and interpreting
system data.
Performing the duties of medical director requires Dr. Brice to be replaced In her emergency
department clinical rotations from time to time. The$30,000 annual cost to the County of this
agreement helps to offset the costs incurred by the Department of Emergency Medicine in
providing physician coverage for Dr. Brice while she is engaged in performing EMS Medical
Directordutles.
FINANCIAL IMPACT: The contract renewal cost far 201 is for$30,000,the same rate as
in the current annual contract. The Managers recommended budget for the Department of
Emergency Management for FY 2002-03 includes funding for this agreement.
NECOMMENDATIONI The Manager recommends that the Board approve the agreement
and authorize the Chair to sign It.

"Cowl)')for provision of EMS Medical Direction Services to Orange County.
WITNESSETH
Wherein,the County and the University have histar eply cooperated and worked
jointly on matters of mutual content and intereat;aM
Whaed,the County operates and maintains a Department ofInnergency
Management which,as one of its functions,provides emergency rankest care to catiants
of the County;and
Whereas,the County wishes to have associated with this program a trained
physician speciali ing in emergency,medicine m provide guidance anal support;and
Whemus,the University,through its Department uf Emageney Medicine,wishes
to provide tide service for the County;and
Whereas,pert ante of the zcdvifim described herein is consistent with the
Umversity's goals of research,[caching,education aad public service.
NOWTREREFORE, inconsiderationofthepamisesa themutualcove is

I. The University agrees to provide a physician fiam the Department of
Emergency Medicine to save as EMS Medical Director to dm County Handles`hhe

2. The duties of the Medical Director shall include the following:

protocols in conjunction with line County Director of Emergency

d. Eacilitam a program of total quality management of patients
addressing compliance to protocol,standards of care,technician
proficiency,appropriate porous management,effectiveness of
treatments end protocols,evaluation for posifive patient
mtcomca end other components of tons EMS system that occur
from access through 911 to delivery(trough to Emetgemy

ongoing wasommendarlons to the Director of system needs and
changes necessary to implement and operate the patient total
quality management Program.
e. Serve as die contact person for UNC Hospitals and r«dve

with thc Director for resolution.
T. Serve on the Orange County EMS Advisory Council.
g. Assist the Medical Affairs Officer for the County's Department
of Emergency Management in general medical issues for HazMat,
Fire, Rescue, Law Enforcement,and others in providing coverage
for special events.Planing for maes casualty,and other activities
outside the routine EMS tasks.
b. Serve as the medical Jvector for the Emegeucy Medical Dispatch

3. The department head of the University's Department of Emergency
Medicine,with the advice and consent of the Director,shall designate the
physician who will serve as the Medical Director and We time of my
