SUBCONTRACT AMENDMENT
This Subcontract Amendment (the “Amendment”) is effective as of the date of last
signature (the “Effective Date”), is by and between by and between the Orange County Health
Department (“Subcontractor”) and the University of North Carolina at Chapel Hill, a North
Carolina non-profit research and education institution with its principal office located at 104
Airport Drive, Suite 2200, Campus Box 1350, Chapel Hill, NC 27599-1350. For purposes of this
Agreement, UNC shall be the Pass- Through Entity (“PTE”).
PTE and Subcontractor may be referred to herein each as a “Party” and collectively as the
“Parties.”
WHEREAS, Parties previously entered into an Agreement for a project conducted under North
Carolina Department of Health and Human Services (NCDHHS) funding,; and
WHEREAS, NCDHHS has extended the prime award period, and, whereas, PTE wishes to
extend the Agreement in accordance with such prime award extension;
NOW, THEREFORE, the Parties agree to the following changes:
There is no change to the Subcontract Number.
4. TERM
4.1 The term of this Subcontract shall begin on the Effective Date and continue until 06/30/2023
(the “Term”).
4.2 Any extension to the Term must be in writing upon terms mutually agreeable to the parties
hereto.
5. COSTS AND EXPENSES
5.1 PTE will decrease funding to Subcontractor in the amount of $21,500.00 in accordance with
Exhibit C (“Budget”)WKHQHZVXEFRQWUDFWWRWDOLV. No other changes to this
section.
