The Museum also plans to develop and offer a new family gallery in this coming calendar year of 2022 and FY of 22-23. In doing so, the Museum will create the only free family and child friendly gallery environment in the county, influencing positive education outcomes and offering young children unique and engaging learning opportunities. In the last two years, the Museum has been growing its reputation through quality exhibits and programs. We are now the only history museum in Orange County (as opposed to the many historic sites). This is a responsibility we take seriously, and in order to bring the community the quality we think it deserves, we would like to transition from three part-time employees to two full-time employees and one part-time employee. This growth in staff will make a tremendous difference in the amount of services we can offer the community, enabling us to increase our reach to include all of Orange County, which is one of our strategic objectives.   DocuSign Envelope ID: 5AAC61C4-23A4-4ADC-A331-E65EC6A2C579 Cover Page Page 9 of 30    To the best of my knowledge and belief all information and data in this application is true and  current.  The document has been duly authorized by the governing board of the applicant.        Tanya Day  Signature:          1/12/2022                           Executive Director  Site Manager    Date    Tonya Brami  Signature:          1/12/2022            Board Chairperson    Date     DocuSign Envelope ID: 5AAC61C4-23A4-4ADC-A331-E65EC6A2C579 Cover Page Page 10 of 30  DISCLOSURE OF POTENTIAL CONFLICTS OF INTEREST AND NON‐DISCRIMINATION CLAUSE  Are any of the Board Members or employees of the agency which will be carrying out this program or  members of their immediate families, or their business associates.    YES     NO (for some reason check boxes are not working for me, so I just wrote in NO)     ☐☐       NO    a) Employees of or closely related to employees of the Town of Carrboro, the Town of  Chapel Hill, or Orange County?         ☐    ☐ NO b) Members of or closely related to members of the governing bodies of the Town of Carrboro,  the Town of Chapel Hill, or Orange County?           ☐     ☐ NO  c) Current beneficiaries of the program for which funds are being requested?                 ☐   ☐ NO  d) Paid providers of goods or services to the program or having other financial interest in the  program?       If you have answered YES to any question, please provide a full explanation below.     NON‐DISCRIMINATION  Provider agrees as part of consideration of the granting of funds by funding agencies to the parties  hereto for themselves, their agents, officials, employees and servants agree not to discriminate in any  manner  of  these  basis  of  race,  color,  gender,  national  origin, age,  handicap,  religion,  sexual  orientation,  gender  identity/expression,  familial  status  or  veterans  status  with  reference  to  any  activities carried out by the grantee, no matter how remote.  The parties hereto further agree in all  respects to conform to the provision and intent of Orange County Civil Rights Ordinance, as amended  and the Orange County Anti‐discrimination Policy.  This provision is enforced by action for specific  performance, injunctive relief, or other remedy as by law provided; this provision shall be binding on  the grantees, the successors and assigns of the parties hereto with reference to the above subject  manner.  To the best of my knowledge and belief all of the above information is true and current.  I  acknowledge and understand that the existence of a potential conflict of interest does not necessarily  make the program ineligible for funding, but the existence of an undisclosed conflict may result in the  termination of any grant awarded.                 DocuSign Envelope ID: 5AAC61C4-23A4-4ADC-A331-E65EC6A2C579 Cover Page Page 11 of 30    Signature:      Tanya Day   1/12/2022                         Executive Director  Site Manager    Date      Signature:      Tonya Brami       1/12/2022                     Board Chairperson    Date    DocuSign Envelope ID: 5AAC61C4-23A4-4ADC-A331-E65EC6A2C579 Program informationPage 12 of 30  AGENCY INFORMATION    Please provide the following information about your agency:    1. Goals to provide free educational history and heritage  opportunities will be accomplished by: presenting two special  exhibits during the FY, designed to enhance the historical  scope of the permanent gallery and represent the County  community; Hosting and/or presenting at least 6 free  programs; Developing a family‐centered gallery to bolster  opportunities for children, especially ages Pre‐K and K, to  improve their education outcomes and have access to early  childhood learning experiences (making the Museum the only  location in Orange County to offer permanent history themed  learning opportunities for children); Providing gallery space for  the Chapel Hill Historical Society to display their items as a  new permanent Museum exhibit.     Tracking of the amount of gallery and programs offerings,  physical and virtual visitor numbers, demographics, and other  analytics will measure how well we perform in providing these  free educational opportunities.  Actual Results  (Outcome)  Ending  FY2021  Two exhibits were displayed, ‘Yésah: Journeys of the  Occaneechi’, and ‘Date Night: Orange County Fashion, Food  Music, & Fun’, both featuring community involvement.   DocuSign Envelope ID: 5AAC61C4-23A4-4ADC-A331-E65EC6A2C579 Outside Agencies/Human Services Program informationPage 18 of 30  Programs offered during FY20‐21 included those associated  with the special and permanent exhibits, with some holiday  themed programs as well‐‐‐in total 13 free programs were  offered during FY 20‐21.   During FY20‐21, in spite of COVID, and with COVID protocols  in place, 752 guests visited the exhibits in person, 296 viewed  the exhibits through virtual tours, and 778 attended virtual  programs. In total, 1,826 individuals engaged with the  museum, which exceeds the original projection of 1,020  guests by 79%.  Projected Results  (Outcome)  Ending FY2022  By the end of FY 21‐22, the permanent gallery will be fully  refreshed and expanded to include a 20th century case. Two  free special exhibits will have been made available to the  public, ‘Date Night: Orange County Fashion, Food, Music &  Fun’, and ‘What’s Your Flavor?!’. In the fall of 2021, a free  program featuring a non‐staff member was presented to the  public related to ‘Date Night’ (‘Date Night’ had 2 other  programs which occurred in FY 20‐21). Additional programs  include Trading Path talks held in late 2021, and various others  planned for early 2022 related to the permanent exhibit.     A family‐centered gallery will be created in early 2022, and will  offer educational displays, handling artifacts, and themed  activities (to be further developed during early FY22‐23 and  beyond, making the Museum the only location in Orange  County to offer permanent history themed learning  opportunities for children). The Chapel Hill Historical Society  will install their first pieces in early 2022.    Per last years application, the projections for engagement for  FY 21‐22 are: on‐site attendance of 500 + ; virtual/in‐person  tours and program attendance of 300+, for a total of at least  1,280 people served. By December of FY 21‐22, we have  already exceeded our on‐site attendance projection, and are  halfway to meeting our program attendance and tour  projection.   Projected Results  (Outcome)  Ending FY2023  Increased community education and engagement about the  history of Orange County measured by individual served by a  10% increase from the number of individuals actually served  during FY 20‐21 (1,828) to 2,010 individuals served during FY  22‐23. In particular, with the development of a family centered  space, we will focus on increasing the number of 0‐18 year  DocuSign Envelope ID: 5AAC61C4-23A4-4ADC-A331-E65EC6A2C579 Outside
