This amendment shall amend the RFQ No. Q12046329, ("the Agreement") by and between Orange
County Criminal Justice Resource Department (hereinafter referred to as "Contractor") and the State of
North Carolina Department of Public Safety, Reentry Programs & Services, (hereinafter referred to
individually as "State"). Unless otherwise expressly amended by the terms of this Amendment, all terms
in the Agreement shall remain the same. The following amendments to the Agreement shall become
effective upon the signatures of all parties being affixed hereto.
The Agreement is hereby renewed for an additional year per the renewal provisions of the Agreement,
effective June 11, 2022 through June 10, 2023.
This Amendment, including all previous Amendments, Agreement and all of its exhibits constitutes the
entire agreement between the parties. This Amendment may be executed in two or more counterparts by
each party, all of which when taken together shall constitute one instrument, it being understood that all
parties need not sign the same counterpart.
The undersigned individuals below represent and acknowledge they have the company power and
authority to bind Orange County Criminal Justice Resource Department and the State of North Carolina
Department of Public Safety, Reentry Programs &Services to the terms of this Amendment.
