Background: No change. II. Purpose: This Agreement Addendum Revision#1 makes funding originally allocated to State Fiscal Year (SFY)22 available in SFY21 to further enhance the Local Health Department's ability to prevent, prepare for, and respond to Coronavirus Disease 2019 (COVID-19)by carrying out surveillance, epidemiology, laboratory capacity, infection control, mitigation, communications, and other preparedness and response activities. Understand that the LHD can provide lodging and wrap-around services as described below for individuals while under state or federal quarantine and isolation orders. 1. Costs associated with testing and monitoring of isolated individuals as well as facilitating isolation to prevent spread of COVID-19. May include wrap around services as necessary including lodging, meals, etc. Support for clinical care is not allowable. 2. Support for social services that will facilitate compliance with isolation and quarantine is allowed. G. If gift cards are purchased by the Local Health Department with the intent of providing them to affected individuals/families to cover these costs, they must be treated like cash with a formal chain of custody system in place. Custody of each card, by"serial"number, must begin when the cards are received by the LHD and be tracked by signature and continue each time a card changes hands until it is received by the client. These records will be subject to audit.
