(`Pethealth') Information Gathering We take the privacy and security of the information we receive from you seriously. At the same time, we have been and continue to be committed to transparency about our collection and use of the information we receive. With these principles in mind, we ask that you review the information herein and then sign the acknowledgment below regarding an agreement to share certain data with Pethealth Services (USA) Inc. as described herein. Background Pethealth, is asking you to share data with them regarding certain information collected from you in your Chameleon/CMS software regarding animals taken in and sent out of your locations. The data will help Pethealth with the data aggregation projects underway for animal welfare as a whole. Specifically, Pethealth wants to obtain this data about animals to assist in its reporting on the aggregate trending in animal welfare organizations and the animals into and out of shelters over time. This reporting will be publicly available and shared with your organization at time of release. For more information, you can contact your Pethealth client services representative. What Does This Mean? This means that the data will be stripped of connectors to any other data points and will not be capable of being linked to or associated with any specific pet. No personally identifiable information of persons will be collected. The data will not be sold, and no published data will single out any specific shelter, regional and state level data will not be shared if one organization constitutes more than 45% of the data set. Should there be any expansion of the use of this data, we will notify you and request additional authorization from you.
