Yes No. If yes, the amended date of substantial completion is: N/A _______________________________________________________________________________________________________________ Full Description of Change: This change request is to add a cedar breather under the new shakes to provide a better lifetime of the new roof section. This also changes the “Heavy (3/4” butt) Treated certified cedar shake” to “Medium 18”x5/8” Medium #1 Tapersawn shakes CCA treated “ An overall inspection of the roofs of all above mentioned buildings will be performed, and any loose, broken and/or missing shingles will be replaced as necessary. Reason for Change: After reviewing the current proposal and reviewing the longivity of the existing system it was determined that a breather was needed in order to prolong the lifetime of the new cedar shakes. Dear Angel, Following our meeting on site about the above-mentioned buildings, we propose the following for your consideration: NOTE: Inspect the existing deck system for signs of damage. (Note: all repairs and/or improvements will be an extra and carried out at our usual “time and material” rates and will be in addition to the contract price.) Not to exceed $1,000.00 plus 7.5% tax per building. Gutter cleaning of several buildings: Several buildings require thorough gutter cleaning and we propose to perform this task at our T&M of $150.00/hr. for two men crew not to exceed $1,600.00 The buildings included are: Historic Courthouse, Norwood Jones Law Office, Dickson House, Doctor’s Office, and Public Bathrooms. Dickson House: The rear South-West roof area has numerous rotten wood shingles that would need replacement. Unfortunately, it does not appear to have been installed per manufacture recommendations and DocuSign Envelope ID: A4033B2B-5D76-4572-B0AA-A2F79ED9F29F Page 2 of 3 could be the reason for this rot. We propose to remove this roof section and replace it with new Heavy (3/4” butt) Treated certified cedar shake shingles fastened with stainless steel nails (equivalent to Hot Dipped Galvanized 153/D) and weaved between each layer with cedar specific 18” felt underlayment. All new 16oz copper flashing to be custom fabricated and installed including an extra custom drip edge. The ridge line will be identical to the original detail (from the French” faitage en lignolet”). Our price for the above outlined work is $12,123.00 plus 7.5% tax ($909.22) for a total of $13,032.22 Norwood Jones Law Office: The South side of this roof shows extensive warping and several shingles have come loose. We propose to replace the entire roof for this building. Similarly, to the Dickson House, we will replace the existing cedar shake shingles with Heavy ¾” butt treated shake, stainless steel nails and 18” felt weaved between each row. All new 16oz copper flashing will be installed around the chimney and new copper drip edge installed. The gutter hangers will be reused and installed under the new shingles. Those gutters will be adjusted as needed during the process. Due to the geographic situation of this building, a special authorization will be needed to close off the street directly under the South side of this building to provide a safe and secure working place, for both, our employees and the civilians traveling by during the reroofing process and to allow for a scaffolding to be installed. Our price for the above outlined work is $13,444.00 plus 7.5% tax ($1,008.30) for a total cost of $14,452.00 Doctor’s Office: The West side of this roof has rotten shingles along with a few split and fallen shingles. We propose to replace this side of the roof only the same way of the previous two buildings mentioned above. Our price for this work is $5,653.00 plus 7.5% tax ($423.98) for a total cost of $6,076.98 DocuSign Envelope ID: A4033B2B-5D76-4572-B0AA-A2F79ED9F29F Page 3 of 3 As noted above, wood or deck repairs and all other improvements changes necessary to yield a proper finish job will be an extra to the contract price. All workmanship associated with this roof will be under warranty for a period of five (5) years from the date of substantial completion. Payment in full is due at completion of project, or as a percentage of completion every two (2) weeks, whichever comes first. Any active account that is over ten (10) days past due will accrue a late charge of .05% per day. Custom material orders for a specific project will be billed and collected before the job can start.
