AMENDMENT TO SERVICES AGREEMENT OVER
$90,000.00 RFP/RFQ

THIS AMENDMENT, made and entered into this the 8th day of October, 2019, by and between the
County of Orange, a body politic and corporate of the State of North Carolina, ("County"), and
Excellance, Inc. ("Provider");
WITNESSETH:
WHEREAS,the County and Provider entered into a Services Agreement dated,February 5,2019,to provide
services to be rendered by Provider to Orange County("Original Agreement"); and
WHEREAS, the County and Provider desire to amend the Original Agreement, while keeping in effect all
terms and conditions of the Original Agreement not inconsistent with the terms and conditions set forth
below.
NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, the
parties agree to amend the Original Agreement as follows:
Amend Paragraph 1. a. Scope of Work as follows:
i) This Services Agreement ("Agreement") is for professional services to be rendered by
Provider to County with respect to remounting two existing Orange County Ambulances
onto Ford Type-1 chassis:
a) Orange County Remount #22-5-2 2310 to include refurbishment as outlined in
Attachment "A", #2252 2310 Orange County Remount Quote — Remount Pricing
Version 18.3, dated 1-8-19.
Amend Attachment#A, Remount Pricing Version 18.3 as follows:
Amend"Original Excellance Unit Number to #2310 instead of#2252.
Except for the change made Paragraph l.a.i)a) and to Attachment A herein, the Original Agreement shall
remain in full force and effect to the extent it is not inconsistent with this Amendment. In the event that
there is a conflict between the Original Agreement and this Amendment, this Amendment shall control.
IN WITNESS WHEREOF, Orange County and the Consultant have signed this Amendment,
effective this the 8th day of October, 2019.
