Do the residents appear neat, clean and odor free? Yes 1. Upon our arrival, the staff were diligently working to feed residents and keep them clean. 3. We witnessed residents being assisted by staff outside to enjoy fresh air and exercise. 2. Did residents say they receive assistance with personal care activities? Ex. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses? Yes 3. Did you see or hear residents being encouraged to participate in their care by staff members? With 4-6 emergency exit doors accessible at all times, two residents complained about the lack of security. This should be addressed at the next visit. Yes Discuss items from “Areas of Concern” Section as well as any changes observed during the visit Follow-up from last visit: Doris addressed the following follow-up issues discovered during previous visits: bed bugs, bathing privacy, and distribution of meds. Doris agreed on the importance of privacy during showers and will make sure all stalls receive a curtain. She mentioned mattresses are preemptively sprayed twice a month for bed bugs. She stated that staff distributes medicines to residents within a two- hour window of when due, and that each resident must finish taking all prescribed meds before staff can begin to distribute meds to another resident.
