PURPOSE: To approve the recommended Home and Community Care Block Grant Far Older
Adults Funding Plan far FY2005-2006.
BACKGROUND: The state Department of Human Resources established the Home and
Community Care Block Grant in July 1, 1992, The grant did not create new funding streams but
combined funds appropriated to the Department an Aging, Department of Social Services, and
Joint Orange Chatham Community Action Agency (JOCCA) to administer programs for low-
income minority elderly and rural elderly,
The Block Grant requires the Board of Commissioners to approve the recommended funding
plan, The funding level estimated by the State for FY2005-2006 is $ 408,977. This amount
represents a five percent decrease from the FY2004-2005 Block Grant final award of $430,502.
"ihe Home and Community Care Technical Advisory Committee composed of representative
from DSS, JOCCA, the DOA, and the County Manager's Office met and reviewed the plan on
April 8, 2005 and supported the continuation of current outreach and services delivery within the
reduced allocation, The Advisory Board on Aging HAS also reviewed the Committee's plan and
recommends approval, The recommended Funding Plan is attached,
FINANCIAL IMPACT: The Home and Community Care Block Grant for FY2005-2006 provides
for a total of $408,977 in state funding to support aging services in Orange County,
RECOMMENDATION(S): The Manager recommends that the Board approve the Home and
Community Care Block Grant Funding Plan for FY2005-2006 in the amount of $408,977 and
authorize the Chair to sign grant documents,

JOCCA
Congregate Nutrition
,I000A has six Congregate Meal Sites located in $98,837 $93,895
Orange County. A site manager is located at each
site and is responsible for the daily operation of
nutrition programs and meals. There are also three
coordinators of services responsible for the
coordination, outreach, referral and provision of
services to low-income families and individuals.
These services include, but are not limited to
weatherization, housing rehabilitation assistance,
financial emergency assistance, Older Workers
Pro ram, and information and referral.
DSS
In-Home Aid Services $ 34,599 $32,869
Home and Community Block Grant enables low-
income, minority individuals to defer or prevent
institutionalization by providing quality services to
meet basic needs. These services include the
performance of essential home management and
personal care tasks rendered under guidance and
su ervision.
Aging

Facility Managers at the three multi-purpose Senior

Transportation
Daily Fixed route transportation is provided far $ 73,394 $73,394
