Who We Are Family Success Alliance (FSA) was formed in 2014 as a collective impact initiative to close achievement and opportunity gaps and end generational poverty in Orange County. We are building a movement to strengthen the pipeline of success from cradle to college and career for our children.We include organizations focused on education, employment,financial literacy, housing, mental health, healthcare and parenting.The parties to this agreement are listed on the signature page;the Orange County Health Department serves as the backbone for this collaboration and is referred to as the "backbone agency' in this agreement. We serve families directly through enrollment in FSA while also serving two larger"zones". See maps for each zone on our website. 2. Shared Values These values are the foundation of our work with families across the collaborative. Every FSA partner and backbone staff person takes action on these values on an ongoing basis: Be Outcome-oriented: We are dedicated to measurable outcomes for children and families. If the data show that we are not effective,we will revise our plans until we get it right. Understand that Relationships are"The Work":We are creating transformativechange built on strong relationships.We first work to change ourselves.Then, through our relationships, we invite change among organizations,families, and systems. Trust the Wisdom of Families: FSA embraces the whole family("Men approach") and aims to be guided by their wisdom. FSA and its partners commit to embedding responsiveness to families throughout our work. Racial Equity and Inclusion: We seek to acknowledge and address the structures that create racial and other forms of inequities in education, health, and wealth. We believe the rich diversity of our families is an asset to be woven into our processes and systems of support. Community Power: Working together with families,we have the chance to uncover our individual and collective power to change circumstances for the better. We believe that our families are experts in their own lives, and together they have the capacity to develop their own solutions to the problems they face. Accountability to the Community: We expect to be in true partnership with families and give them the power to drive our work.We strive to create a supportive space where we can hold each other accountable to this ideal. Partner Mutual Expectations & Responsibilities: The following responsibilities are required of partners to ensure the effectiveness of the collaborative: All Partners: • Provide services and support that furthers the long-term mission and vision of all children in the zone graduating from high school prepared for college and career and the empowerment of their families and communities • Commit to using an equity and asset-based approach when working with families and planning services. Engage in ongoing discussion to strengthen our understanding and implementation in this area. • Refer eligible children,youth and caregivers to partner agencies when appropriate in order to provide comprehensive services to each family. • Assist in promotion, marketing, and outreach of FSA as opportunities arise. • Ensure that appropriate staff attend regularly scheduled meetings of the collaborative to discuss grant implementation, service coordination, and strategic direction. • Submit reports and provide additional information requested by the fiscal agent and backbone agency in a timely manner, including paperwork required for receipt of funds. • Participate in agreed upon processes that ensure accountability to services and supports committed to FSA- enrolled families and to deliverables outlined in FSA-related work plans, including reporting outlined in "FSA Collaborative Partners:Accountability& Reporting", incorporated by reference. • Agree to the budget allocation reported in the Funding Agreement for the current fiscal year. Funding allocations will be based in criteria agreed upon and discussed by partners with recommendations from the backbone agency. Amendments to the Funding Agreement will be discussed with partners for review. • Ensure appropriate child safeguarding protocols are in place when providing services to children, including the provision of childcare during adult-oriented programs, and agree to consider adoption of a formal child safeguarding policy, such as any relevant policy adopted by the FSA Advisory Council and incorporate into MOU by reference. • It is also recommended that all partner agencies be covered by General Liability insurance that includes sexual abuse/molestation coverage. Fiscal Agent: • The Orange County Partnership for Young Children has fiscal monitoring responsibilities for funds dispensed by United Way of the Greater Triangle, as follows: (1) review of partners' quarterly budget to actual reports and (2) review of partners'fiscal year audit. Backbone Agency: • Lead efforts to ensure a shared vision for change, including a common understanding of the problem and a joint approach to solving it through agreed upon actions. • Facilitate agreement on common measures, collect and analyze relevant data, and evaluate results to ensure outcomes for families are achieved and partners can hold each other accountable. • Coordinate partners to establish a plan for implementing mutually-reinforcing activities that help families achieve their goals and is consistent with the resources available in the Funding Agreement or with resources potentially available through fundraising efforts. • Communicate consistently and openly to build trust, assure mutual objectives, and appreciate common motivation. DocuSign Envelope ID:C83972F6-1100-4600-8747-8D3DE320234B The Family Success Alliance collaborative is also accountable to its Advisory Council.The Council advises and makes non-binding advisory recommendations on resource allocations, program planning, and policy changes to ensure a pipeline of high-quality services to reduce the detrimental effects of poverty for children and families living in the County. More information about the Advisory Council is available on the FSA website. Failure to meet responsibilities outlined above will be addressed by the backbone agency with the partner.The partner agency will have the opportunity to provide input to bring the agency into compliance. Continuous and egregious failure to come into compliance will be addressed with the partner agency and the FSA Advisory Council, and the backbone agency may terminate the partner's participation in FSA through this MOU, subject to any Funding Agreement. 4. Fundraising • Inform FSA backbone agency of potential funding opportunities and/or assist in grant proposal writing to help sustain FSA's collective viability. • Consult with FSA backbone staff prior to submitting any grant application that proposes activities specifically intended to benefit FSA, FSA partners, or FSA families. • Utilize FSA partner talking points in grant proposal writing and in agency communications related to fundraising that is FSA-related • Review proposals in their entirety prior to submission, if the specific partner organization is included in a larger proposal by FSA to potential funding sources. S. Date, Term and Authorized Partner Signature This agreement will become effective upon signing by all parties and will commence at the start of the funding award, beginning July 1, 2019 and extending through June 30, 2020 The agreement shall be reviewed annually at the beginning of each fiscal year by the parties to determine its continuation and/or need for modification. At any time, a partner agency may terminate its participation in FSA under this MOU by giving 30 days written notice to the lead agency,subject to any Funding Agreement. Payment will be distributed through Electronic Funds Transfer. EFT's will allow us to process allocation, designation, and vendor payments in a timely manner. Please fill out the EFT form(attached) in its entirety. For Collaboratives Only: The Fiscal Agent is responsible for distributing payments to the other partners in the collaborative. The mutual accountability requirements for your collaboration should be employed during the funding year to identify any potential need to adjust individual agency payments or to make changes in the list of DocuSign Envelope ID:C83972F6-1100-4600-8747-8D3DE320234B funded partners during the year.The collaborative partners as a group -- not solely the fiscal agent—are responsible for decision making regarding payments to partners. If changes are made during the year, a revised Funding Agreement will be signed. United Way does not or will not require the fiscal agent to assume any responsibility for financial monitoring or oversight of the partner agencies regarding their use or handling of United Way funds. Performance, outcomes and activities are the collaborative's responsibility for execution and the collaborative must be accountable to the partnership members and United Way. Reporting Timeline • July 8, 2019- Deadline to submit completed and signed Funding Agreement • February 2020- Mid-Year Progress Report—This will be a qualitative check-in. • July 2020-Year-End Report for 2019-2020-This includes performance outcomes and demographics for households. Marketing/Storytelling expectations for Community Impact Partners • Make your best storytellers available for UWGT staff to learn from and harvest stories that will help advance the mission and goals of United Way. • Collect client success stories and participate in the creation of promotional videos and other materials. • Community Impact Partners, as well as UWGT, will cross-post events on social media (Facebook, Twitter) related to our work together. • Display the approved United Way of the Greater Triangle logo prominently in your office or service facilities. • Use the approved United Way of the Greater Triangle logo on primary publications and materials for programs supported by United Way funding (i.e. newsletters, organization brochures, annual reports or posters). • Add the approved United Way logo and a link to United Way of the Greater Triangle's homepage (www.unitedwaytriangle.org)to your web site where it is visible. • Non-Disparagement Clause—Agree to not disparage the name of United Way of the Greater Triangle or any of its employees, board members or volunteers in their roles at United Way of the Greater Triangle in the media (broadcast, print, mobile/text or social media outlet). Volunteer Engagement Engaging UWGT's donor base in meaningful volunteer experiences is key to the success of our issue awareness building and fundraising efforts. Your funding proposal was partly evaluated on the quality and types of volunteer experiences presented and we are excited to work together to provide these activities in the coming year. UWGT staff will work with the appropriate volunteer engagement staff at your organization in order to schedule, plan and execute the experiences. Best Practices for Individual Agencies • Provide a calendar of agency special events you would like UWGT staff to attend. Indicate any sponsorship opportunities you would like to submit for consideration. • Acknowledge United Way support in any publication/program where other sponsors or contributors are listed such as annual reports.
