Re: Health Services Agreement
Dear Board of County Commissioners:
We understand Orange County is considering adding a FITIMAT program at the Orange County
Detention Center. After discussions with Orange County Detention Center representatives and
Dr. Wade, our Medical Director, this letter serves to confirm Dr. Wade would agree to provide an
additional one hour on-site per week at an additional cost of $2,460.00 annually ($205.00 per
month) effective July 1, 2019 to assist with servicing this County program. This would result in a
new annualized base contract price of$380,205.36 ($31,683.78 per month).
We kindly ask the County to provide a signature below to keep on file acknowledging the
County's agreement to the terms as above-stated. Please keep this fetter for your records and
