Do the residents appear neat, clean and odor free? yes When we arrived, about 6 residents were congregating in the communal area and were dressed, clean and interacting with he staff and each other. 3 were coloring with staff assistance and 3 were watching tv. Others were in their rooms, most were napping but also appeared neat and dressed 2. Did residents say they receive assistance with personal care activities? Ex. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses? Yes 2. all residents appeared to be neat and clean when we arrived at the residents. A few residents were napping or in their rooms, but they appered to be dressed as well 3. Did you see or hear residents being encouraged to participate in their care by staff members? Yes 3. Staff were activity engaged with residents 4. Were residents interacting with staff, other residents & visitors? Yes 4. Three residents were participating in a craft activity when we arrived, a volunteer joined them about 11:30 and sung with 6 residents 5. Yes A volunteer arrived at 11:30 to lead an activity, It was reported by staff that there is a variety of volunteers who visit and lead activities, prayer etc. 20. Keys were left in the lock of the door to the room medications were stored, 2. A planter was used to prevent a rear exit door from being opened, in order to prevent a resident from eloping or wandering out that door. yes Discuss items from “Areas of Concern” Section as well as any changes observed during the visit We noted that the residence was incredibly clean, odor free, residents looked like they received great care, and appeared to be happy. We also discussed: 1. The door to the medication room was locked, but staff keys were left in the lock. Staff removed the keys once it was brought to their attention. They indicated that they left them in the room as they had to return to document glucose levels just monitored. 2. The need to develop other methods of securing the back door that will prevent elopement, but not egress if there is a fire This Document is PUBLIC RECORD.
