BACKGROUND: The Orange County Health Department was awarded one-time competitive fronds totaling $19,710 for FY 2004-2005 from the NC Department of Health and Human Services (DHHS) to support a project that would increase physical activity far seniors. Ellen Young, the Health Promotion Coordinator and representative to the Active Living by Design Grant Project awarded to the town of Chapel Hill, pulled together a planning group that included the Orange County Department on Aging and the Triangle J Council of Governments. This project will bring together public health educators, land use planners, recreation planners, and senior services professionals to work with senior citizens and other local residents to promote physical activity in the neighborhood surrounding the planned new Southern Orange County Senior Center, the existing Southern Human Services Center, and Homestead Community Park. Other agencies and organizations such as Cooperative Extension may participate as the project develops further. Project outputs as detailed in the attached proposal submitted to DHHS include the formation of community residents' project teams, walkability/bikeability assessments, development of a Walking Trail Map, development of a toolkit for project replication, and a demonstration Project Report with recommendations and findings. TJCOG will be responsible for the following project outputs: • Conducting a review of all relevant local plans and preparing a summary document of findings by .January 2005. • Preparing a GIS-based map of current and planned facilities and potential physical activity sites in a radius to be determined around the planned Southern Senior Center by January 2005. • Participating as a project leader in the selection of an assessment tool and the development of a community training to conduct the assessments in the neighborhood surrounding the Southern Human Service Center by March 2005. • Compiling and analyzing the results of the community assessments, developing a report, and distributing to Project Team members by April 2005. • Participating with the Project Team in formulating recommendations and preparing the final report, including graphics and layout of the final report by April 2005. • Creating draft of trail map and producing a final trail map for publication by May 2005. • Participating in the creation of the Active Seniors Project Toolkit by .June 2005. The agreement with TJCOG is for a total of $16,540, all to be paid from the grant. The remaining grant funds will be used by the Health Department to support the project outcomes. FINANCIAL IMPACT: The one-time award of $19,710 will support the costs of the project, including the agreement with TJCOG for the services of a Land Use Planning Manager, who was part of the application team. No additional County funds are anticipated for the support of this one-time project. RECOMMENDATION(S): The Manager recommends that the Board approve the agreement, subject to final review by staff and the County Attorney, and authorize the Chair to sign. AN AGREEMENT BETWEEN ORANGE COUNTY HEALTH DEPARTMENT AS A PART OF THE COUNTY OF ORANGE AND TRIANGLE J COUNCIL OF GOVERNMENTS THIS AGREEMENT, effective November 1, 2004 through June :30, 2005 by and between the Orange County Health Department, hereinafter referred to as OCHD and Triangle J Council of Goverrunents, hereinafter referred to as TICOG. The Orange County Health Depaztment is part of the County of Orange. WITNESSETH: WHEREAS, T.ICOG was an active partner in the development of the Active Seniors Project as submitted to the North Carolina Depaztment of Health and Human Services by the OCHD, and WHEREAS, T7COG and the OCHD agreed that the OCHD would act as the fiscal agent for' the Active Seniors Project in accordance with the requirements of the North Carolina Department of Health and Human Services, Division of Public Health, and WHEREAS, OCHD has been awarded one-time funding to carry out the elements of the Active Seniors Project, and WHEREAS, TICOG desires to provide services as outlined in the Active Seniors Project Proposal, under the terms and conditions set forth in this Agreement and consistent with the intent authorized by the funds provided by the North Carolina Division of Public Health, NOW, THEREFORE, in consideration of the premises and of the following mutual promises, covenants, and conditions, the parties agree as follows: L TJCOG will perform the following activities in an effort to increase the opportunities for physical activity for residents using or living in the areas around Homestead Park, the Southern Human Services Center and the anticipated Southern Orange Senior Center: Conduct a review of all relevant local plans and prepare a summary document of findings by .Ianrtary 2005. Prepare a GIS-based map of current and planned facilities and potential physical activity sites in a radius to be determined around the planned Southern Senior Center by Ianuary 2005. Participate as a project leader in the selection of an assessment tool and the development of a community training to conduct the assessments in the neighborhood surrounding the Southern Human Service Center by March 2005. 4 • Compile and analyze the results of the conununity assessments, develop a report, and distribute to Project Team members by Apri12005, • Participate with the Project Team in formulating recommendations and preparing the final report, including graphics and layout of the final report by April, 2005. • Create draft of trail map and produce a final trail map for publication by May 2005. • Participate in the creation of the Active Seniors Project Toolkit by .Tune, 2005. 2. T.1COG will expend the grant funds as agreed on the following items: • $15,860 for technical support of the Land Use Planning Manager • $100 for GIS analysis map plots • $250 for GIS trail map plots, framed trail reference maps and handout maps • $180 for travel 480 miles X $0..375/mile • $50 for research documents • $100 for the production of CDs for the project tool kits 3, TJCOG will expend funds by Apri130, 2005. 4, T.1COG will submit a project report by December 31, 2004 and March 31, 2005 that includes progress on activities to date and total expenditures to date by line item. TJCOG will submit an end of project report by June 30, 2005. The final report will include a program activity report and a final expenditure statement, Copies of receipts for materials purchased will be included in the final report. 6, TJCOG will submit an invoice for the full agreement amount no later than April 30, 2005. 7. OCHD agrees to provide financial support to the T.1COG in an amount not to exceed $16,540 for the Active Seniors Project, payable within thirty (.30) days of OCHD's receipt of an invoice. 8, This Agreement shall become effective on November 1, 2004 and continue in effect through June 30, 2005. 9. This Agreement contains the entire understanding of the parties and shall not be altered, arnended or modified, except by an agreement in writing, executed by the duly authorized officials ofboth parties. 10, The laws of the State of North Carolina shall govern the validity and interpretation of the provisions, terms, and conditions of this Agreement. Project Description A. The key recommendations that this project will address from the N C. Blaeprint and Moving Our Children Toward a Healtha~ Weight include: i. Increasing opportunities for physical activity by fostering supportive policies and environments. a. Increasing the numbei of facilities and/or environments that promote plrysical activity. b. Increasing the number of policies, practices, and incentives to promote physical activity. ii. Providing more community-based opportunities for leisure-time/recreational physical activity for all children and youth. iii. Creating an environment that makes active lifestyles the norm rather than the exception, iv. By doing # i, this project will help increase public awareness of the importance of physical activity and the need for supportive policies and environments. B. The specif c target population is senior citizens and other neighborhood residents, including children., C. This project will bring together public health educators, land use planners, and senior services professionals to work with senior citizens and other local residents to promote physical activity in the neighborhood surrounding a planned new Orange County Senior Center, an existing human services center, and an existing community park. Orange County is in the early stages of plamring a new senior center in Chapel Hill. The project site is close to a human services center, a community park, a future planned greenway, retail development, several churches, residential neighborhoods, and other potential destinations, but there are a number of obstacles that currently inhibit safe travel to these places on foot or by bike. This project will organize senior citizens and other neighborhood stakeholders to systematically identify these obstacles and reconmiend solutions to improve the safety and quality of the local pedestrian and bicycle network in order to help seniors and others be more physically active. This is a unique opportunity to influence the site design of the senior center, as well as improvements that can be made in the surrounding neighborhood. More than 200 senior citizens and county employees will use the Senior Center and work at the Human Services Center every day, on average. Over the course of a year, an estimated 3,000 different seniors will use the Senior Center. In addition, more than 7,500 residents live within one mile of the project area.. As a result, an extensive population will be served by the project interventions. ARer recruiting the Active Seniors Project Team, project staff will hold a brief training to teach participants how to use a pedestrian and bicycle infrastructure assessment tool, such as the Walking & Bicycling Suitability Assessment (WABSA) developed by the iJNC School of Public Health and NC DHHS, in order to systematically evaluate the local pedestrian and bicycle network. The Project Team will then conduct this assessment of the pedestrian and bicycle infrastructure in the area around the planned new senior center and existing community park, and develop recommendations for needed improvements. In addition, the Project Team will identify existing routes for walking and biking, to the extent they exist, that seniors and others can use to be physically active. Project staff will then produce a "Trail Map" that can be posted and distributed at the Senior Center, Human Services Center, and Community Park to help publicize these loops. This will ensure that the project has an immediate impact on helping local residents access walking and biking facilities at the same time that it identifies recommendations for how to improve these facilities and connecC them with the larger pedestrian and bicycle network in Chapel Hill. The Project Team's findings will be summarized in an illustrated report and PowerPoint presentation, and presented to the Senior Center Design Committee, the Town of Chapel Hill, Orange County, and others to help inform future infrastructure investment decisions.. In addition, project staff will compile a tool kit of the materials used in the project to make the lessons learned available to other communities in North Carolina. In this way, the project will help publicize existing pedestrian and bicycle facilities around the Orange County Senior Center and Homestead Community Park, catalyze needed improvements, and serve as a demonstration project to help inspire others to create more walkable communities for senior citizens and all residents in their community. D. The expected outcomes of the interventions are the mapping of existing walking routes and the identification of infrastructure improvements needed to create a safe and continuous pedestrian and bicycle network in the area surrounding the plam~ed Orange County Senior Center, existing Human Services Center, and existing Homestead Community Park. The project will also create an organized constituency that is empowered to advocate for the fiurding and construction of these improvements, and share its experiences with others. 2. Statement of Need Seniors are one of the fastest growing populations in Orange County and are at greater risk for chronic diseases. Physical activity is a critical part of a healthy lifestyle. The Division of Aging and Adult Services, - NC Departrnent of Health and Human Services, recognizes this and, has identified health/welhress and fitness as a component of a new senior-friendly commwrity initiative. Orange County is in the early stages of plamring a senior center to be located at the Southern Human Services Center Complex in Chapel Hill that is near Homestead Community Park, a community park with walking paths. In recent completed surveys (117) and public forums attended by approximately 150 older adults regazding construction of a new senior center in Orange County, older adults advocated for more outdoor activities i,e. walking paths, tennis courts, and gazdening, Survey respondents ranked the ideas of fitness and wellness as number 1 and number 3 out of a possible top ten priorities for the new senior center.. While walking is not the only path to address the need and desire for outdoor fitness, it is certainly recognized by a growing number of seniors and many health care professionals as one of the best forms of daily exercise. Currently there are a number of'obstacles that inhibit safe travel to the park and local businesses. They include safely crossing Homestead Road to get to the park (with hazards including speeding high-school students coming from Chapel Hill High off' Seawell School Rd) and finding a continuous linked pedestrian network to walk along.. At a number of points, the sidewalk dead ends abruptly as it passes from one development to another in the area around the park. By advocating for changes to improve the walkability of the area, the Active Seniors Project could potentially benefit seniors, employees of the Southern Human Services Complex, residents of the Brookstone apartments and other community members. 3. Collaboration and Capacity The project will bring together public health educators fiom the Orange County Health Department, senior services professionals from the Orange County Depaztment on Aging and the Triangle .1 Area Agency on Aging, local senior advocates wlro are helping to plan the design for the new senior center, and land use plamrers from the Triangle Council of Governments. In so doing, the resulting Active Seniors Project Team will have a greater opportunity to promote physical activity in the neighborhood surrounding the planned new Senior Center located on Homestead Road.. Members from the Project Team will solicit input from their respective constituencies as the project progresses. Other key community leaders who will share in the planning of this project are the OC Advisory Board on Aging, and the Chapel Hill Parks and Recreation Commission.. The project team will work with existing local entities to gather the necessary information prior to formulating a pedestrian and bike assessment training tool and administer that to the surrounding community.. ~o Once the project assessment is completed, findings and recommendations will be formulated and presented to the key stakeholders and decision makers in the community, including the Senior Center Design Committee, OC Department on Aging Advisory Board, City and County Land Use Planners, Chapel Hill Parks and Recreation Commission, Chapel Hill Town Board, Orange County Board of Commissioners, etc. The recommendations identified in the project will also be placed on electronic venues for the public.. It is the project team's plan that the findings in this assessment will influence the municipal and county decision makers to find a way to implement the recommendations identified in the project. A toolkit will be developed to inform other communities of this effective way of drawing together collaborative agencies and constituents to better address barriers to fitness proximate to community facilities, including a senior center. In conclusion, this project will create a dynamic interdisciplinary partnership of aging service providers, public health professionals, and land use planners that is uniquely qualified to respond to the challenge of promoting greater activity for older adults. In the process, it will improve the pedestrian and bicycle infrastructure for everyone in the community. Statement of Assurances If awarded funds by the Physical Activity and Nutrition Branch to implement community-based interventions to promote healthy eating and physical activity in children, their families, and their communities, the local health department agrees to: ^ Use funds for community-based, multi-level interventions that focus on implementation of the recommendations, goals and objectives put forth by Eat Smart, Move More.,,North Carolina and the NC Healthy Weight Initiative. ^ Partner with appropriate community organization(s) to plan and implement the project. ^ Implement the proposed activities according to the time frame outlined in the application. ^ Submit information on activities conducted with grant Rinds through the PAN Progress Check System. ^ If appropriate for the local intervention, grantees are encouraged to use the Physical Activity and Nutrition (PAN) Behaviors Monitoring Form for collecting and reporting data on heights weights, and physical activity and eating behaviors.. As of July 1,2004, this form will be available as a screen in HSIS to record this data on health department clients over two years of age, Counties that do not use HSIS can add the new form to their system or they can log into HSIS to access the form. Afree-standing system should be available in the fall of 2004 to allow non-health department settings to use the form. ^ Provide a written Summary of activities and outcomes by August 2, 2005 using the very brief format provided by the Community Change Success Stories.doc. This form will be sent to funded applications. ^ Share experiences, challenges and successes with other communities through presentations, newsletters, community change chronicles and other media as needed.
