("Provider"); WITNESSETH: WHEREAS,the County and Provider entered into a Services Agreement dated,May 22, 2018, and identified by the County as 2018-194-E Housing, to provide services to be rendered by Provider to County's Housing Department ("Original Agreement");and WHEREAS, the County and Provider desire to amend the Original Agreement, while keeping in effect all terms and conditions of the Original Agreement not inconsistent with the terms and conditions set forth below. NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, the parties agree to amend the Original Agreement as follows: Add the delivery of an Asbestos Operations & Maintenance Initial Class III Training to Sections 1 a.i), Scope of Work, and 3 a.i), Basic Services; and increase the maximum amount payable for Basic Services to $22,050 in Section 5 a., Compensation for Basic Services, to include the cost of the Training services, which is $7,050. Except for the changes made Section la.(i), Scope of Work, Section 3a.(i), Basic Services, and Section 5a. herein, the Original Agreement shall remain in full force and effect to the extent it is not inconsistent with this Amendment. In the event that there is a conflict between the Original Agreement and this Amendment, this Amendment shall control. This 16-Hour course is designed for operations and maintenance personnel who may be required to complete small scale removal of asbestos. It includes demonstrations using glovebag techniques and mini-enclosures. A course manual will be provided, and trainees will Iearn the health effects of asbestos, proper respiratory protection, asbestos regulations and control options,and methods of waste disposal. The O&M program can be divided into three types of projects: ■ Those that are unlikely to involve any direct contact with ACM. These projects may involve routine cleaning of shelves and counter tops or other surfaces in a building(provided ACM debris is not present). Generally, such activities would not be expected to disturb ACM. • Those that may cause accidental disturbance of ACM. These projects could include maintenance work above a suspended ceiling in an area that may have surfacing ACM overhead. • Those that involve relatively small disturbances of ACM. These projects--small-scale, short-duration maintenance, repair, or installation projects involving minor disturbances of ACM—include activities such as installation of new light fixtures on or in an ACM ceiling. A single glovebag operation to remove a small amount of ACM to repair a pipe in a boiler room is another example of intentional small-scale, short-duration disturbance. Cost for Asbestos O&M Initial class III Training for up to 10 attendees: $2,800.00 After 10 students, 11-20 students would be bilked at a rate of$250.00 per student Any student after 20 will be billed at a rate of$125.00 per person. This proposal is valid for fifteen(15)days from the date of submission. An invoice for the full project amount will be submitted following course completion.
