PURPOSE: To approve an agreement and receive funds from UNC Hospitals far partial
support of Healthy Carolinians Coordinator position,
BACKGROUND: The Healthy Carolinians of Orange County process began work in 1995 with
an extensive community assessment, The County, the UNC School of Public Health, and the
UNC Hospitals were all contributing financial partners in this initial effort, The Board of
Commissioners authorized the creation of the Healthy Carolinians Coordinator position during
the FY 1999-2000 budget deliberations as a Health Department position and agreed to support
the position and operational expenses at a 50% level, This authorization was contingent upon
the ability of the Health Department to secure funds for the remaining 50% of the cast of the
position. The position was first filled in June of 2000 with the following combination of funds:
50%from the county; $10,000 from the state of North Carolina; and $11,122 from the University
of North Carolina. The state funds and the funds from UNC comprised 50% of the salary and
benefits for the position, Recnaitment for the original position took six months and the position
was vacant far five months in 2001 -from July 2001 until November 2001, The current
Coordinator, Maria Hitt, has been in the position since that time.
In 2003, three years of decreasing grant funds were secured from the Kate B, Reynolds
Foundation in partial support of the Healthy Carolinians Coordinator position. For FY 2003-
2004, Foundation support was $20,000; for FY 2004-2005 Foundation support is in the amount
of $15,000; and the final year of Foundation support will be in FY 2005-2006 with an amount of
$10,000,
For the current fiscal year, the salary and benefits for the position are a total of $49,337 and are
being founded by the following sources: $15,000 from the Kate B, Reynolds Foundation funds;
$10,992 from UNC Hospitals; and $24,668 from the County general fund, The positive balance
of funds will cover the 2% cost of living increase authorized for October 11. This is one-time
funding, but is the second time UNC Hospitals has been able to identify funds in support of this
position, In past years, UNC Hospitals have provided funds or in-kind support for a significant
number of project activities. This agreement allows the county to accept the funds from UNC
Hospitals.
Significant accomplishments of this effort and the coordinator in just the past two years include:
• Completion of the 2003 Community Health Assessment with participation from more than
700 County residents,
• Publication of Orange County Community Health Assessment Executive Summary.
• Securing grants for partners in specific high priority areas. Youth tobacco prevention
project; milk machines in schools, six mini-grants to community agencies, teen
pregnancy prevention grant funds, funding for coordinator position, support for family fun
and fitness day.
• Collaboration on publication of county-wide recreational map.
• Collaboration on child abuse and prevention community agency trainings and brawn bag
lunch series,
• Developed and conducted the first Family Fun and Fitness Day in October 2003 in
Chapel Hill,
• Published health information for employers in the Chapel Hill Chamber newsletter.
• Successful completion of statewide recertification of Healthy Carolinians in 2004.
• Coordinates five committees each with 3-25 members in high priority areas identified in
the community assessment,
FINANCIAL IMPACT: For the current fiscal year, the salary and benefits far the position are a
total of $49,337 and are being funded by the following sources: $15,000 from the Kate B.
Reynolds Foundation funds; $10,992 from UNC Hospitals; and $24,668 from the County

RECOMMENDATION(S): The Manager recommends that the Board approve the agreement,
accept the funds in the amount of $10,992 for FY 2004-2005 from UNC Hospitals, and
authorize the Chair to sign the agreement subject to final review by staff and the County
Attorney,

THE ORANGE COUN'T'Y HEALTHDEPARTNIENT
FOR HEALTHY CAROLINIANS OF ORANGE COUNTY
THIS AGREEMENT, made effective July i, 2004 by and between The University of North
Carolina Hospitals, hereinafter referred to as "UNC Hospitals", and Healthy Carolinians of Orange
County, a program managed by the Orange County Health Department as pan of the C,-ounty of
Orange, hereinafter referred to as "Healthy Carolinians",
WITNESSETH:
WHEREAS, Healthy Carolinians seeks financial support for the Healthy Carolinians
Coordinator position.
WT-TEREAS, UNC Hospitals desires to provide funds to the Orange County Health
Department for the sole purpose of supporting the Coordinator position for Healthy Carolinians
under the terms and conditions set forth in the Agreement and consistent with its mission in
providing communityservice.
NOW THEREFORE, in consideration of the premises and of the following mutual
promises, covenants, and conditions, the parties agree as follows:
1. Healthy Carolinians Coordinator performs the following activities in an effort to
improve the health status of Orange Countyxesidents:
A. Staff Healthy Carolinians Council of Orange County, including regularly
scheduled meetings of the Council and of the Executive Committee.
B, Assist committees (Advocates for Children, Advocates for Adolescents ,Healthy
Choices C.,ommunity Voices) in planning, implementation and evaluation of
activities,
C. Provide health data analysis.
D, Provide grant-writing to support community health activities,
E, Provide media facilitation and public education for annual events and special
events.
F. Represent Healthy Carolinians on appropriate committees and other
partnerships,
G. Work with other initiatives in the county to facilitate communication,
coordination of events and efficient use of county resources.

2. LTNC Hospitals agrees to provide financial support for the Healthy Carolinians
C..oordinator in an amount not to exceed $10,992.00, payable within thirty (.30) days of
ZINC Hospitals' receipt of an invoice from the Orange C..ounty Health Department.
3. This agreement shall become effective,July 1, 2004 and continue in effect through June
30, 2005.
4. This Agreement contains the entire understanding of the parties and shall not be altered,
amended or modified, except by an agreement in writing, executed by the duly
authorized officials of both parties.
5. The laws of the State of North Carolina shall govern the validity and interpretation of
the provisions, terms and conditions of this Agreement,
