Yes Resident Profile Yes/No N/A Comments/Other Observations (please number comments) 1. Do the residents appear neat, clean and odor free? Yes 1- We were pleasantly surprised by the facility cleanliness and improvements since the last visit. Residents observed and interviewed were neat, tidy and odor free. 2-5. Residents were anxiously waiting for their names to be called, as monthly stipend checks were being distributed in the dining hall. Consequently, they weren’t too interested in interacting. Talking with some residents was difficult due to cognitive and communication challenges. 2. Did residents say they receive assistance with personal care activities? (i.e. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses) Yes 3. Did you see or hear residents being encouraged to participate in their care by staff members? Yes 4. Were residents interacting with staff, other residents & visitors? Yes 5. Did staff respond to or interact with residents who had difficulty communicating or making their needs known verbally? Did residents describe their living environment as homelike? No 8. Although it was not described as homelike, residents said their rooms were good and they had what they needed. 9. There were no odors detected, which is a big improvement since our last visit. 12. Smoking areas outside. Residents are allowed to come and go freely. 9. Did you notice unpleasant odors? No 10. Activities are posted and some efforts are made to welcome activity feedback and ideas. 15a. Monthly activities are printed in large font on the bulletin board outside the dining room. 17. Meals are planned, and portions controlled according to regulations. The administrator has personally been cooking her favorite seafood dishes for the residents. At their discretion, residents can go offsite for meals. 17b. Residents expressed satisfaction with food options and quality. 19. A signup sheet was available for residents interested in meeting with Pinnacle bank about opening their own checking account. 15a. Was a current activity calendar posted in the facility? No 15b. Yes 16a. Can residents access their monthly needs funds at their convenience? (#16 and 16a pertain only to residents on Medicaid/Special Assistance. NHs $30 per month. ACHs $66 minus medication co-pay and full cost OTC drugs) Yes 17. Are residents asked their preferences about meal & snack choices? (Adult Care Home residents should receive snacks 3X per day. Yes No Areas of Concern Exit Summary Are there resident issues or topics that need follow-up or review at a later time or during the next visit? Areas of Prior Concern: Feces odors and cleanliness issues Present Areas of Concern Staff said they are concerned about their current occupancy rate. Nearly 80% of residents are on Medicaid. Large potholes in the parking lot could be hazardous to cars and residents walking offsite. They don’t own a power generator, but contract out when one is needed. They don’t have any handicap accessible automatic doors. Discuss items from “Areas of Concern” Section as well as any changes observed during the visit. Give summary of visit with Administrator or SIC. Does the facility have needs that the committee or community could help address? Yolanda and her husband own Crescent Green and she freely shared their challenges and recent successes. The staff has worked hard in recent months to clean and improve the facilities. We were pleasantly surprised by the great strides they’ve made since our last visit. Yolanda was actively making preparations for hurricane Florence expected to arrive Thursday. She was communicating with the fire department and other local assisted living facilities about their emergency plan.
