Yes Resident Profile Yes No N/A Comments/Other Observations (please number comments) 1. Do the residents appear neat, clean and odor free? Yes 1-7. All residents appeared healthy, comfortable, and positive. At the time of the visit, residents’ doors were shut and few residents visible. Staff told us they just finished exercise classes and meetings and were preparing for dinner. There were more staff visible than residents. The staff was professional and caring when interacting with residents. The residents we spoke with provided no feedback on how to improve the living conditions. They were very positive about their experience and enjoyed living at Carol Woods. 2. Did residents say they receive assistance with personal care activities? (i.e. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses) Yes 3. Did you see or hear residents being encouraged to participate in their care by staff members? Yes 4. Were residents interacting with staff, other residents & visitors? Yes 5. Did staff respond to or interact with residents who had difficulty communicating or making their needs known verbally? Did residents describe their living environment as homelike? Yes 8-14. The facility was in good order and felt comfortable overall. No medical, cleaning supplies or hazardous materials were visible in the hallways or located on carts. One hallway with no windows only had half the lights turned on. It may have been an electrical issue or simple oversight. 9. Did you notice unpleasant odors? No 10. Did you see items that could cause harm or be hazardous? Yes 10a. Were unattended med carts locked. N/A 10b. Were bathrooms clean, odor-free and free from hazards? Yes 10c. Were rooms containing hazardous materials locked? Yes 11. Halls were properly lit and filled with artwork. Safety instructions and residents rights were clearly identifiable. Photos of staff on duty are displayed for residents, guests, visitors with names clearly printed. 19. Not only does the staff post a calendar of events, but they provide a community wall showcasing fliers for specific upcoming gatherings, movies, and shows. 15a. Yes Yes Areas of Concern Exit Summary Are there resident issues or topics that need follow-up or review at a later time or during the next visit? Most residents were unable to meet with us because they were transitioning between their exercise routine and preparing for dinner. The residents we did speak with didn’t raise any issues or ideas on how to improve their experience. They were very happy with the facilities, staff and living conditions at Carol Woods. Discuss items from “Areas of Concern” Section as well as any changes observed during the visit. Give summary of visit with Administrator or SIC. Does the facility have needs that the committee or community could help address? ______________________________________________ Consistent with previous visits, Carol Wood provides one of the counties best residential experiences for the senior community. At the exit interview, we discussed how the staff is preparing for the arrival of hurricane Florence on Thursday. They had multiple meetings to review their emergency policies and procedures.
