Call bells trigger a loud alarm that could be heard on the entire unit (prompting them to be addressed quickly). 9. Did you notice unpleasant odors? no 10. Were residents asked their preferences or opinions about the activities planned for them at the facility? no 15. Activities are planned by the activities director. Though they are varied, residents interviewed stated they were not asked about preferences. It was suggested perhaps this could be addressed at resident council meetings. 17b. Lack of meal choice was mentioned by several residents. A standard meal is served and an alternative is available if the resident prefers something else. The rep we spoke to noted some have meals brought in. One family member noted that the meal was not always appropriate for residents; e.g., more ‘finger food’ would be helpful. One mentioned the same thing was always served for breakfast though it was not what she preferred, and a new tray often needed to be prepared. 15a. Family Council? yes Areas of Concern Exit Summary Are there resident issues or topics that need follow-up or review at a later time or during the next visit? - Several comments concerned meals: lack of choice, lack of ‘finger food,’ and food quality. - A family member complained that it seemed there was a lack of communication between shifts regarding needs (bed alarm, and the fact that the resident needed to be fed). - A concern was expressed about clothing not coming back from the laundry or being returned to the wrong resident. Discuss items from “Areas of Concern” Section as well as any changes observed during the visit. Give summary of visit with Administrator or SIC. Does the facility have needs that the committee or community could help address? Exit was held with Admissions Coordinator. -Family members appreciated being given the phone number of a nurse advocate at the admission meeting. One mentioned she had called with concerns which were addressed. - Facility was clean and staff members were consistently friendly and helpful. - A part time activities director coordinates activities every day of the week; residents are invited to participate at the time of the activity. Activities range from coloring, jewelry making, and language instruction to manicures, bingo, socials, and religious services. - Several residents were observed to be enjoying sitting outside (these were ones who appeared able to get there on their own). - Greatest concerns were about meals. 1/2015
