8. Did residents describe their living environment as homelike? Yes 11. At times a loud sound (alarm?) could be heard
in the hallways. A staff member told us it was to
alert staff that a resident was in the restroom.
13. One resident noted that though staff were
caring, sometimes it could take up to 30 minutes
for a call bell to be answered. Another resident
stated bells were answered promptly.

15. Were residents asked their preferences or opinions
about the activities planned for them at the facility?
Yes 15. Many residents were attending activities
during our visit. A large print board posted
activities for the day; monthly boards were
displayed in hallways and residents also had
paper copies. The activity room was reflective
of ongoing activity such as jigsaw puzzles,
crafts and Valentine Day preparations.
17. Positive comments were made about
meals. Residents are offered menu
alternatives. They are encouraged to eat in the
main dining room.
19. The activity calendar lists visits from
religious communities and therapy dogs.

Areas of Concern Exit Summary
Are there resident issues or topics that need follow-up or
review at a later time or during the next visit?
The administrator mentioned that the current state report
had recently been received and he had not yet placed it in
the notebook. The trash overflow issue, state in the report,
was partly a result of inclement weather cancellation/
delays and had been resolved by increased pick up from
two to three days a week.
Discuss items from “Areas of Concern” Section as well as
any changes observed during the visit. Give summary of
visit with Administrator or SIC. Does the facility have
needs that the committee or community could help address?
The administrator mentioned there were unfilled C.N.A.
positions due to lack of qualified candidates. He discussed
statewide trends of fewer people entering and completing
programs.
As renovations are made, more single rooms are included to
accommodate resident wishes.
Several residents expressed appreciation for physical and
occupational therapy. One family member, a physician,
noted her mother had made much progress since she had
come to Brookshire and could not think of any concerns.
There have been no cases of flu at Brookshire. Three
Hospice partnerships are welcomed at Brookshire: UNC,
Duke, and Alamance County. Recently a program to honor
residents who had recently passed away was well received.
The administrator has worked on updating safety plans.
