One resident mentioned she sometimes had to wait before someone could help her with personal needs. Residents expressed appreciation for staff. 5. One nursing assistant found hearing aids for a resident so that she could communicate. 2.Did residents say they receive assistance with personal care activities? (i.e. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses) Yes 3. Did you see or hear residents being encouraged to participate in their care by staff members? Did residents describe their living environment as homelike? Yes 9. An unpleasant odor was noticed in one area. The administrator noted this was likely a result of health issues of a few of the residents. 10a. One drawer on a health cart was unlocked. The nurse immediately noticed when it had been opened. 13. and 14. Call bells were accessible. Some mentioned that at times they had to wait to have needs met. During our visit, the nursing director responded quickly when resident pushed the bell. Aids were seen making sure bells were accessible. 9. Did you notice unpleasant odors? Yes 10. Were residents asked their preferences or opinions about the activities planned for them at the facility? N/A 15. Many residents were gathered in the activity room looking forward to manicures given by the activities director and her assistant. They mentioned enjoying art and jewelry making. A collage of photographs was displayed of resident activities including a vegetable garden. A monthly activity calendar was in each resident’s room. 17 Residents expressed satisfaction with dining choices and quality. Each morning they complete menus and are giving several choices. A recent change in schedule where residents who opt to dine in the dining room are served first has resulted in increased participation 17c. Residents are offered cold water or cranberry juice along with a snack. 19. Schools, religious groups, and health care training programs are involved at Brookshire. 20. The administrator often informs the resident council of upcoming issues and seeks input. Efforts to form a family council have been unsuccessful. 15a. A followup visit may be scheduled to learn more about the activities program. Discuss items from “Areas of Concern” Section as well as any changes observed during the visit. Give summary of visit with Administrator or SIC. Does the facility have needs that the committee or community could help address? A warm and welcoming environment was evidenced by friendly greeting from many staff members. A housekeeper stated how much he enjoyed the people at Brookshire and his dedication to the residents. The nursing supervisor was visible and make it a point to welcome us and learn the name of a new member of the committee. When talking about the resident council, the administrator explained how he planned to invite residents to participate in staff hiring committees. The administrator appeared to be interested in comments and observations. He confirmed a concern expressed by one resident (food items served that the resident should not eat) and how it had been quickly addressed. He was especially pleased with a new activity director who he felt had greatly enhanced the program.
