1. Do the residents appear neat, clean and odor free? Yes 1-5) The residents were very clean and
appeared well cared for. We observed
them interacting with staff on several
levels—personal care, opinions,
friendly discussion. They interacted as
possible with our committee. The
staff interactions we observed were
appropriate and high quality with staff
meeting the residents at their function
level.

activities? (i.e. brushing their teeth, combing their hair,
inserting dentures or cleaning their eyeglasses)

8. Did residents describe their living environment as homelike? NA 8. The residents on site at our visit are
living with dementia and describing
their environment was not likely,
however, they were smiling, friendly
and appeared comfortable, secure and
relaxed.
11. Yorktown is very quiet.
13. Buttons in the bathrooms and
bells available for those that want/need
them. Unfortunately, most residents
are no longer capable of using the
bells to get help. Yorktown has bed
and chair alarms for those that need
them.

activities planned for them at the facility?
Yes 15 + 15b. Staff reported they observe
residents’ interest in activities and
build activity programs around what
the individuals enjoy. Two or three
activities are offered each day
including music, visits, exercise.
Several of the residents attend the
Charles House on various schedules
during the week. (See #20)
17. With only 6 residents, person-
centered care is the focus for care. We
observed snacks that appear to be
available during the day.
17a,b,c. Dining is family style in the
comfortable and attractive open
dining/family room. If a resident does
not want to join a meal at the table
they are welcome to eat
when/wherever they'd like, usually the
choice is in a recliner.
Residents are given choices, they may
go offsite with proper supervision, or
alone if they are able.
19. Visitors are welcome and
encouraged, including inviting girl
scout troops (etc.) to meet at the
house, neighborhood children and pets
to visit. UNC Students share activities.
20. Family meetings semi-annually
and a "ladies circle" monthly to
discuss the month’s activities and their
monthly dinner (ordered in).

At the interview, JudyWoodall shared concerns about staffing.
The program currently has one caregiver opening. Other staff
shifts have been adjusted to fill Yorktown resident care needs
as the Charles House works to fill the job. This is a current and
growing problem for many care programs. Qualified, trained
staff in short supply as care for elders grows.
Discuss items from “Areas of Concern” Section as
well as any changes observed during the visit. Give
summary of visit with Administrator or Supervisor-In-
Charge. Does the facility have needs that the
committee or community could help address?

We visited with Brittany and Judy Woodall (Charles
House RN who supervises health care at the 3
programs).The facility is very attractive, clean,
orderly, and comfortable. The home is currently all
women. There are 4 private bedrooms and 1 double
bedroom. 2-persons share each of the well-designed
bathrooms. The bedrooms are cozy and very nice,
furnished with personal belongings Most of the
residents are elderly (one celebrated her 100th birthday
this year); however, another person with MS is in her
60’s. Residents have a range of care needs, physical
and cognitive strengths.
