Everyone was up, dressed neatly, moving around, watching TV or visiting with family. 2.Did residents say they receive assistance with personal care activities? (i.e. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses) Yes 3. Did you see or hear residents being encouraged to participate in their care by staff members? Were residents asked their preferences or opinions about the activities planned for them at the facility? Yes 15a. Activities calendar has mostly passive activities. Calendar is located on front wall in plain sight. Residents reported that the holiday celebrations and dinner were good. 17a.There is a nicely decorated dining room table that is always set-up for eating. Due to the need for some residents to be fed, they prefer to have all residents in same location when dining. 19. Some of the residents work at OE Enterprises, which has opportunities for persons with disabilities and/or vocational rehab. Several of the activities on the calendar indicate that they will be conducted by persons from outside organizations, such as local area pastors and churches. Residents reported that churches, family & friends delivered presents at Christmas. 15a. Was a current activity calendar posted in the facility? Yes 15b. Were activities scheduled to occur at the time of your visit actually occurring? N/A 16. Do residents have the opportunity to purchase personal items of their choice using their monthly needs funds? No Areas of Concern Exit Summary Are there resident issues or topics that need follow-up or review at a later time or during the next visit? Due to the change in ownership, we will want to monitor how the care might change over time. Discuss items from “Areas of Concern” Section as well as any changes observed during the visit. Give summary of visit with Administrator or SIC. Does the facility have needs that the committee or community could help address? Gutters were being cleaned at time of our arrival. Sanitation Grade of 100 clearly posted. The Resident Rights posted in the back hallway has the wrong name listed as Ombudsman but the one in the front is correct. **The biggest change is that on November 7th, Louise sold Villines Rest Home to Adorable Senior Living. She is acting administrator at this time but will retire soon. We commented to ourselves and to her that she was as relaxed as we've ever seen her and was quicker to smile. No staff was lost as a result of this changeover.
