No Resident Profile Yes No N/A Comments/Other Observations (please number comments) 1. Do the residents appear neat, clean and odor free? No 1. Most residents were cleanly dressed with appropriate attire for the weather. Two women walking down a hall had a strong urine odor. 2. One resident (with limited movement) who wished to remain anonymous indicated that he is unable to obtain assistance during the night to empty his urinal. He then has no choice but to wet the bed and remains so until the morning. He does not have a ‘call bell’ and if he calls out, no one comes. He also reports that his wheelchair tipped over last week when he was outside. No one at the facility noted this but he was assisted by passing motorists who stopped to help him. He is aware of his resident’s rights and has the ombudsman phone number memorized. He indicates that when he calls, the phone just rings and no one ever answers. 3. The same resident as above indicates that staff, rather than ‘encouraging’ him to participate in his own care, demands that he does so, though he is physically incapable of completing the task without assistance. 4. Very few staff members were seen, except when notifying residents to collect for dinner. One staff observed passing meds in the dining room; another doing laundry. 2. Did residents say they receive assistance with personal care activities? (i.e. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses) Yes 3. Did you see or hear residents being encouraged to participate in their care by staff members? No 4. Were residents interacting with staff, other residents & visitors? Yes 5. Yes 15. One resident indicated that she had no idea that there were any activities offered at the facility. 15a. One resident indicated that someone periodically comes to play the piano and that they had BINGO that day. There were no activities during the visit. 17. One resident indicated that they are provided snacks in the morning, afternoon and night. 17b. Two residents agreed that they are unable to get more than one cup of coffee or tea during meals. They also stated that they have to arrive in the dining hall at 5:00 PM but are not served until 5:45PM. 17c. There is no fresh ice water available. 15a. Was a current activity calendar posted in the facility? No 15b. Were activities scheduled to occur at the time of your visit actually occurring? No 16. Do residents have the opportunity to purchase personal items of their choice using their monthly needs funds? Yes 16a. Can residents access their monthly needs funds at their convenience? (#16 and 16a pertain only to residents on Medicaid/Special Assistance. NHs $30 per month. ACHs $66 minus medication co-pay and full cost OTC drugs) Yes 17. Are residents asked their preferences about meal & snack choices? (Adult Care Home residents should receive snacks 3X per day. Nursing Home residents should be offered snacks at bedtime.) No 17a. Yes No Areas of Concern Exit Summary Are there resident issues or topics that need follow-up or review at a later time or during the next visit? Areas of Prior Concern: -Strong urine odor down one hall -Bathrooms with wet towels -Inadequate meal portions Present Areas of Concern -Lack of nighttime monitoring -Strong urine odor on one or two women residents -Inability to get additional beverages at meals Discuss items from “Areas of Concern” Section as well as any changes observed during the visit. Give summary of visit with Administrator or SIC. Does the facility have needs that the committee or community could help address? The committee met with Kevin Crawford, Resident Care Coordinator, at the beginning of the visit. Mr. Crawford shared welcome news about how Crescent Green had begun using the QuickMAR electronic medication administration system. This program allows for easier management and administration of pharmaceuticals, and reduces medication administration errors and provides real-time verification of prescriptions being administered. Mr. Crawford also indicated that he is using the program to upload residents’ medical record information for the residents. He indicated that this process change has been extremely beneficial to the facility in the two months since its rollout, with most staff buying in and a noticeable reduction in paperwork (this was observed by the committee, who usually sees a large stack of papers on Mr. Crawford’s desk, which was not present at this visit). Mr. Crawford also indicated that the facility has hired a new administrator, Ms. Doris Coleman. Additionally, there is an attempt to hire CNAs to improve knowledge and skill of employees and to promote professionalism and continuity. The facility census has moved up somewhat, and although challenges with PCS qualification still persist, Mr. Crawford indicated that things are stabilizing somewhat. To follow up with issue (see #2 above), the Ombudsman’s number was called by a Resident Advocate the next day (Friday). Although the phone was not answered, there was an opportunity to leave a message. A return call to the RA was received on Monday and the resident’s concerns discussed. The Ombudsman was familiar with this resident and indicated that she would investigate the matter.
