No Resident Profile Yes/No N/A Comments/Other Observations (please number comments) 1. Do the residents appear neat, clean and odor free? Yes 1-Most residents observed and interviewed were neat, tidy and odor free. 2-5. Less staff interaction was noted at this visit than at recent visits, possibly related to the shift change and staff meeting. Most staff interaction seems to be related to care tasks. At our site visit, talking with residents was difficult due to some residents’ cognitive and communication challenges. 2. Did residents say they receive assistance with personal care activities? (i.e. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses) Yes 3. Did you see or hear residents being encouraged to participate in their care by staff members? No 4. Were residents interacting with staff, other residents & visitors? Yes 5. Did staff respond to or interact with residents who had difficulty communicating or making their needs known verbally? Did residents describe their living environment as homelike? No 8. Although it was not described as homelike, residents said their rooms were fine and they had what they needed. 9. The exit area of the 200 wing had a notable odor of feces. We were told the shift change was underway and staff routinely take care of incontinent tasks as needed and at shift changes. A brief staff meeting had pulled many staff off the floor slightly delaying the diapering. 12. Smoking areas outside. 9. Did you notice unpleasant odors? Yes 10. Were rooms containing hazardous materials locked? Yes 11. Did residents feel their living areas were kept at a reasonable noise level? Yes 12. Does the facility accommodate smokers? Yes 12a. Where? Were residents asked their preferences or opinions about the activities planned for them at the facility? Yes 15. Activities are posted and some effort is made to offer opinions about activities. 15a. Activities were posted for three days handwritten, in large font on the bulletin board outside the dining room. 17. Meals are planned, and portions controlled according to regulations. Meal choice is limited, however the people visited today did not have complaints. We observed the afternoon snack cart sharing snacks. NOTE: Crescent Green is an “open facility” and many residents go offsite where they may purchase food and beverage choices. 17c. A large cooler of ice water is available with disposable cups at a table in the dining room. 15a. Was a current activity calendar posted in the facility? No 15b. Were activities scheduled to occur at the time of your visit actually occurring? No 16. Do residents have the opportunity to purchase personal items of their choice using their monthly needs funds? Yes 16a. Can residents access their monthly needs funds at their convenience? (#16 and 16a pertain only to residents on Medicaid/Special Assistance. NHs $30 per month. ACHs $66 minus medication co-pay and full cost OTC drugs) Yes 17. Are residents asked their preferences about meal & snack choices? (Adult Care Home residents should receive snacks 3X per day. Areas of Prior Concern: Bedbug infestation Present Areas of Concern A non-resident who has caused some problems for one of the residents was banned from the facility and he showed up. Staff saw the interloper and notified staff who called Carrboro Police. It was resolved quickly but raises a possible facility security issue. Although the Director states Crescent Green is not a locked Memory Facility, when talking with residents, various cognitive challenges are evident. She said residents “sign in and out.” They watch for wandering behaviors and if it becomes a safety issue, residents are moved to an appropriate facility. Discuss items from “Areas of Concern” Section as well as any changes observed during the visit. Give summary of visit with Administrator or SIC. Does the facility have needs that the committee or community could help address? Doris Coleman was not on site, but on vacation. Yolanda was open about Crescent Green and shared information freely. Due to shift change and a brief staff meeting, fewer staff were observed in the public areas at today’s site visit.. Yolanda said that limited funds are available for facility improvements (e.g. painting and other improvements), but they are working on improvements as available. We noticed residents having difficulty entering and exiting the building due to the heavy doors, mobility issues, wheelchairs, and no automatic door openers. Yolanda was interested in the possibility of an automatic “handicap” door opener on one of the doorways.
