A considerable amount of resident- staff interaction was observed. 5a. Most administrative staff had nametags, though direct care staff did not. 2. Did residents say they receive assistance with personal care activities? (i.e. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses) Yes 3. Did you see or hear residents being encouraged to participate in their care by staff members? No 4. Were residents interacting with staff, other residents & visitors? Yes 5. Did residents describe their living environment as homelike? Yes 8. Multiple residents had very personalized rooms and commented on feeling at home. 11. The facility was very quiet overall with the positive exception of a guitar performance in one of the activities rooms. 13. Some residents had call bell pendants and one individual indicated that once summoned, staff typically respond quickly. 9. Did you notice unpleasant odors? No 10. See note 11 above (a guitar performance was underway during committee’s visit and drew a good size audience). 17. An active Food Committee meets to provide facility input about dining choices. Multiple residents expressed very favorable opinions about meal options. 17a. Residents can dine in the café area or in their rooms. 19. Guitar player performs monthly. 20. Facility has an active Activity Council and Food Council. Additionally, there are monthly family meetings. 15a. Was a current activity calendar posted in the facility? Yes 15b. Give summary of visit with Administrator or SIC. Does the facility have needs that the committee or community could help address? The committee met with Executive Director Laurie Sawyer upon entry (she was busy assisting a new admission upon exit, at which time we met with the activity director). Ms. Sawyer, and in fact all staff with whom we interacted, were exceedingly friendly, helpful and welcoming. These interactions contributed to an overall very positive visit. We learned that the issues they were having with the flu have subsided special precautions are no longer necessary. Census remains low, and appears to be related to efforts to provided primarily private room accommodations to residents. Very few even minor complaints were brought to the attention of the four committee members visiting. The facility was clean, and residents were either out and about, or resting comfortably in their rooms. Memory Care residents were seemingly quite pleased to have visitors and shared quite a lot about their positive experiences at Carillon. Resident and family involvement in dining and activities appears to be strong and valued. The committee learned of an upcoming mystery theater event for residents, families and the community in which staff will be the actors. Many staff members conveyed a seemingly very sincere degree of care for their residents.
