1. Do the residents appear neat, clean and odor free? Yes
1.-2: All residents interviewed in
assisted living second and third floors
said that the care they receive is good.
One said that staff is friendly, and that
the care provided is good to excellent
and staff responses to help are good.
There was training going on in the
memory care unit, but one resident
was available to visit and she was very
pleased with Brookdale in all respects.
She referred to a special activity that
all residents enjoyed that morning.

the facility, in rooms, halls, sitting
areas, and gathering spaces. Staff was
also visible and interacting with
residents.

homelike. As usual, there were
attractive seasonal and themed
decorations throughout. Resident
rooms into which visitors were invited
were clean and neat

and some observations indicate that
the new activities and programs are
welcomed and well received. The
activities appear very creative, current
and interesting. As we left the facility
a staff member was playing the piano
and singing with residents joining him
in the first floor sitting room.

Brookdale is providing good care to residents, has a well-
received food menu and dining, and has a strong activities’
program in which many residents participate.
Discuss items from “Areas of Concern” Section as
well as any changes observed during the visit. Give
summary of visit with Administrator or Supervisor-In-
Charge. Does the facility have needs that the
committee or community could help address?
In the exit interview, the visit team met with Allison
Mihaly, Executive Director. Once again, positive
comments from residents indicate Brookdale is
working hard and succeeding at providing a good
program and a good home for their residents.
