Areas of Concern Exit Summary
Are there resident issues or topics that need follow-up or
review at a later time or during the next visit?
1. Correct Ombudsman name on resident’s rights
poster.
2. Ensure no hazardous materials are kept in the
unlocked closet.
3. Ensure staff wear name tags.
Discuss items from “Areas of Concern” Section as
well as any changes observed during the visit. Give
summary of visit with Administrator or Supervisor-
In-Charge. Does the facility have needs that the
committee or community could help address?
Committee met with Ms. Ogbonna pre- and post-
visit, in addition to Louise Murphy, Administrator
upon exit. They were both present throughout the
majority of our visit (given that it was dinner time
and it’s a small facility), so were privy to much of
the discussion. Generally, we conveyed that the
visit was positive and most residents reported
satisfaction with their care and environment.
