Please read carefully and sign below
Orange County Animal Services (OCAS) agrees to the following;
1. The animal you will be fostering has been:
a. Given a complete physical exam.
b. Vaccinated with all age appropriate vaccinations
c. Examined for internal parasites, if age appropriate, and treated if positive.
d. Tested for feline leukemia if age appropriate (if applicable)
2. OCAS makes no guarantees, claims, representations, or warranties, either express or implied, as to the behavior,
health, temperament or training of the above-described animal(s).
3. OCAS retains legal ownership of all foster animals, reserves the right to contact and/or visit the foster home at any
time regarding the welfare of the foster animal, including taking possession of the foster animal if the animal is
not returned to OCAS when scheduled
The Foster Volunteer agrees to the following:
1. I agree to provide the animal(s) with the following basic requirements;
a. Provide adequate food and water in order to maintain normal health and weight
b. Provide clean and safe shelter within a safe and comfortable temperature range
c. Provide kind and gentle treatment at all times, and include this pet as a member of the family
2. I agree to the ensure that cats will remain indoors only
3. I agree to notify OCAS if I believe the animal requires veterinary attention. OCAS will determine if any medical
care will be performed on the animal. The foster parent may at his/her own expense, and with prior approval from
the Veterinary Health Care Manager, provide veterinary intervention.
4. I agree to notify OCAS of any address changes
5. I agree to notify OCAS if at any point I decide that I can no longer foster the animal.
6. I agree to not allow the animal to be used for medical or other experimental purposes
7. I agree to transport this animal within a cat carrier in an enclosed vehicle.
8. I agree to indemnify, defend, and hold harmless OCAS, Orange County, and any of its officers, officials,
employees, or agents from all losses, liabilities, claims, demands, suits, costs, damages or expenses (including
reasonable attorney’s fees) arising from bodily injury, including death, to any person or persons or damage to
property caused in whole or in part by any negligent or intentional act on the part of myself or occurred to or
suffered by myself, my household, or any third parties directly or indirectly related to activities arising out of this
Foster Contract.
9. I agree to not promise the animal to anyone or to imply the authority to approve a potential adoption
10. I agree to return this animal for all scheduled medical appointments
11. I agree to return the animal(s) when scheduled to for them to be placed up for adoption.
12. I agree to comply with all federal, state, and local laws, ordinances, rules, and regulations applicable to the care
and keeping of animals.

It is agreed that Orange County Animal Services (OCAS) retains superior title to the above –described animal(s).
Should there be any non-compliance with this contract or if the above foster care provisions are not being afforded this
animal, OCAS may immediately take possession of this animal and legal action may be taken against the foster parent.
