THE ORANGE COUNTY BOARD OF HEALTH MET ON THURSDAY, JULY 22, 1999 AT THE SOUTHERN HUMAN SERVICES CENTER IN CHAPEL HILL, NORTH CAROLINA. CALL TO ORDER AND INTRODUCTION OF GUESTS Dr. Ives called the meeting to order at 7:45 p.m. Rosemary Summers introduced the staff present. II. ADOPTION OF AGENDA (July 22, 1999) AND APPROVAL OF MINUTES (June 24, 1999) A. APPROVE AGENDA (July 22, 1999) and MINUTES (June 24, 1999) The agenda of July 22, 1999 was approved. The motion to approve the July 22, 1999 agenda was made by Beverly Foster, seconded by Jonathan Klein and carried without dissent. The minutes of June 24, 1999 were approved. The motion to approve the minutes of June 24, 1999 as stated above was made by Barry Adler, seconded by Ron Cammarata and carried without dissent. III. HEALTH DIRECTOR’S REPORT A. NC HEALTH CHOICE Orange County is at 88% enrollment with only 57 children left to “find” to be at the State’s 100% level. Michele Easter, an Americorp volunteer with Interfaith Council, wrote and received a grant to purchase materials to support outreach at the Buckhorn Flea Market with the Latino population. Cathy Ferniany, the Health Department Health Check Coordinator, will be coordinating with Ms. Easter. The Health Department will be distributing flyers at all Orange County Schools for the first days of school. The State will facilitate a re-enrollment process for those already enrolled and due to be re-enrolled beginning October 1. The dental benefits bill did pass the legislature and became available for clients July 1. KERR DRUG INITIATIVE Kerr Drug is co-sponsoring the Health Department Senior Vaccination Season Kick-Off Event at University Mall on October 2 and 3. Flu and pneumonia shots will be given by Health Department nurses. Pharmacy students or pharmacists will provide a “brown bag” review of senior’s medications. The Health Department will staff a weekly site during October at Kerr Drug in addition to the regular senior sites. There was concern from the Board regarding the time frame flu shots will be given. The Board has suggested beginning the flu shots from the middle of October through the middle of November and do an educational piece around late August/early September on the appropriate timing for the vaccinations. C. ASTHMA COALITION Stephanie Willis of the Chapel Hill / Carrboro School System has taken on organizing this coalition. A meeting was scheduled on July 15 to further discuss this effort. Stephanie wrote a grant application for a small amount of funds to start some work in the schools. She asked for the Health Department’s concurrence and co-signature on the application so that we can keep a coordinated approach. The full coalition will be meeting in early fall to more fully develop a targeted plan. There may be small amounts of money available through the State Environmental Health office targeted at asthma triggers in homes that could be available this spring. The plan is to integrate this work into the Needs of Children Committee for Healthy Carolinians. D. HEALTHY CAROLINIANS Non-recurring funds were allocated for the program by the legislature. The State is still determining how that money will be allocated and distributed. The potential is there to fund a portion of the Healthy Carolinians Coordinator. The position descriptions are in Personnel and waiting for funding before starting recruitment. Duke Endowment should notify us in August regarding implementation funding. The BRFSS data is complete. The Board should get a look at it during the Board work session in the fall. E. CRIMINAL JUSTICE PARTNERSHIP PROGRAM ADVISORY BOARD This is a new board that is a combination of the Juvenile Advisory Board and the Criminal Justice/Alternative Sentencing (CJPP/TASC) Boards in the county. This is a joint board with Chatham County. Rosemary Summers has been serving on the CJPP/TASC Board and Eileen Kugler has served on the Juvenile Board as the Health Department is mandated to be represented. Rosemary Summers has been asked to serve as co-chair of the planning and assessment committee. Rosemary’s plan is to use the assessments already conducted to the extent feasible and blend this planning process into already existing community planning. F. IFC HOMELESS SHELTER There are discussions beginning regarding the use of the County land at the Southern Human Services Center site. The County does have a plan for that piece of property and will be looking at what needs to be located there. If the Homeless Shelter does locate on the property it could have profound impacts on the Health Department’s service delivery. For example, the Health Department does not currently offer adult primary care services to this population, rather that is done through a voluntary effort of providers at the current shelter. It would make little sense for that model to continue at a new shelter, when a medical facility is close-by. OZONE ALERTS The County is now part of the NC Air Awareness Coalition. The new ERCD has taken the lead on this as part of the Ozone Air Quality action plan the Board of County Commissioners asked them to pursue earlier in the year. One part of the program is the receipt of e-mail alerts one day in advance of projected code orange or code red ozone days. David Stancil, the ERCD Director discussed this with Rosemary Summers and they agreed that the alerts will be posted by the Health Department. We will work on accompanying the alerts with educational pieces on clean air. H. INTENSIVE LIVESTOCK OPERATIONS Ron Holdway attended this seminar and came away informed as to how these issues are being handled and what the impact could be for Orange County. The hog population has risen from 2,500,000 to 10, 000,000 since 1990. This brings up several issues for Orange County as there are 3 major hog farms in the County. Several counties already have rules / ordinances. The lagoon phase out proposed by the Governor has been removed. The moratorium on new operations is still intact until 2001. These issues should be discussed by the Board at a future meeting and decisions made as to whether or not Orange County should pursue having rules / ordinances. I. HEALTH DEPARTMENT WEB PAGE The Health Department will be “on-line” in late August with an extensive web page. Access is through the County web page which is: www.co.orange.nc.us. J. RECRUITMENT Recruitment letters were sent to 694 RN/BSNs. The Clerk’s Office reports they have received about 10 phone calls requesting an application, more than they have ever had for any County Board. Rosemary Summers has also received informational calls. Another mailing to master’s level and above nurses should go out in early August. IV. ACTION ITEMS A. ON-SITE WASTEWATER RULES A draft of the proposed wastewater rules along with a synopsis of the changes were presented to the Board at the June meeting for review. Included in the Board packet for the month of July was the summary and explanation of the proposed changes, along with a full text draft with current and proposed local modifications. The County attorney recommends changing Section .1945 Available Space from a 3-bedroom requirement to: For sites on an existing tract of land to be subdivided or for any lot created after the effective date of these rules, available space shall be based upon the square footage of nitrification field required for a minimum design wasteflow of 360 gallons per day for the long- term acceptance rate determined in accordance with Rule .1955, or .1956(1) of this Section. This will also apply to (b) in Section .1945. The motion to approve the amended On-Site Wastewater Rules was made by Brenda Crowder-Gaines, seconded by Jonathan Klein and carried without dissent. WTMP EXPANSION PROPOSAL Included in the Board packet was a recommendation that the Board approve the WTMP Expansion Plan and forward it to the Board of County Commissioners for approval to move forward with the educational plan as Phase I of the Expansion Plan. Phase II of the Plan will be developed during 2000-2001. Also included in the packet was the executive outline of the WTMP Expansion, WTMP Educational Plan and the WTMP discussion paper. The Board approved adding a utility provider partnership. The motion to approve the WTMP Expansion Proposal and forward it to the Commissioners as stated above was made by Janet Southerland, seconded by Brenda Crowder-Gaines and carried without dissent C. GRANT APPLICATION FOR ORANGE COUNTY HEALTH AWARENESS PROJECT The Orange County Partnership for Young Children has made additional funds available for new projects in Orange County beginning October 1999. This project targets preschool children who remain at home and those in more informal care-giving settings and seeks to link them with community services that will enhance their health and well-being. A particular focus will be on the Latino population in these settings. The project calls for funding a position entitled “Bilingual Community Awareness Coordinator” to develop an effective community health awareness and health education campaign targeted to young families. The project appears to conform to a desire by the Board of County Commissioners that Orange County Departments be more cognizant of the need to serve the Latino population; it is a first step in implementing recommendations contained in the Los Ninos Report the Board heard in May; and fits with established health department functions in linking citizens to needed services. The motion to approve the Health Department to forward the Grant Application with the Orange County Partnership for Young Children for Community Health Awareness Project to the Board of County Commissioners was made by Janet Southerland, seconded by Brenda Crowder-Gaines and carried without dissent. V. DISCUSSION ITEMS A. WATER AND SEWER BOUNDARY TASK FORCE REPORT Included in the Board packet was the final version of the Water and Sewer Management, Planning and Boundary Agreement. The staff has reviewed the final version of the Water and Sewer Boundary Task Force Report and listed several issues that were included in the packet. The Environmental Health staff wou ld need to be involved actively in the factual determinations, however the “remedies” are all planning remedies. The Board would like to request a “visual” report as the written report was unclear. The Board is concerned with the decision-making and flow of information to the public. Barry Jacobs would like to direct the concerns / issues directly to the Planning Department. It was suggested that a joint meeting of the Planning Board and the Board of Health take place. The motion to receive the report and pursue additional information regarding the Water and Sewer Boundary Task Force Report was made by Brenda Crowder-Gaines, seconded by Jonathan Klein and carried without dissent. ADVERSE PUBLIC HEALTH CONDITION The concept of the rule was first envisioned as defined in the Water and Sewer Boundary Task Force Report. The definition was broadened and developed into a rule that might apply to situations other than water or sewer applications. The Board considered the rule last spring and requested consultation with the county attorney. After discussions with the attorney at the October Board meeting, the rule was delayed for consideration pending further work and clarification on the final task force report. After further consultation with the attorney, the Health Director is recommending that the Board drop the Adverse Public Health Rules from consideration at this time. The definitions and remedies that would be available to the Health Department for enforcement of this rule would be confusing as it relates to the application contained in the Water and Sewer Boundary Task Force Report. That report contains these definitions with planning and zoning remedies attached and should not be confused with health rules and enforcement. The motion that the Board not pursue further development or adoption of the Adverse Public Health Condition Rules at this time was made by Barry Adler, seconded by Mel Hurston and carried without dissent. VI. MOVE TO CLOSED SESSION: PERSONNEL MATTER (Authority G.S. Section 143-318.11 (a) (6)) The motion to move into Closed Session: Personnel Matter (Authority G.S. Section 143-318.11 (a) (6)) was made by Brenda Crowder-Gaines, seconded by Ron Cammarata and carried without dissent. VII. MOVE TO OPEN SESSION AND ADJOURN The motion to adjourn from closed session and return to open session and approve minutes of the closed session was made by Barry Adler, seconded by Jonathan Klein and carried without dissent. The motion to adjourn the meeting was made by Brenda Crowder-Gaines, seconded by Janet Southerland and carried without dissent. The Board of Health meeting adjourned at 10:45 P.M.
