THE ORANGE COUNTY BOARD OF HEALTH MET ON December 20, 2007 at the Southern Human Services Center, Chapel Hill, North Carolina. Welcome and Introductions Chris Harlan, Vice-Chair, called the meeting to order at 7:15 P.M. in the absence of the Chair. II. Approval of December 20, 2007 Agenda The motion to approve the Agenda of December 20, 2007 was made by Moses Carey, seconded by Jim Stefanadis, and carried without dissent. III. Public Comment for Items NOT on Printed Agenda None IV. Action Items on CONSENT Agenda A. Minutes approval of November 13, 2007 meeting. B. Smart Start Preschool Project Coordinator Contract Renewal The Smart Start Preschool Screening Project has utilized a dental hygienist on part time contract to coordinate the scheduling of childcare providers and “screeners” for the project. In addition, this contract position handles all of the data entry as a result of the screenings and prepares a summary report of findings. The contract contains no new provisions from previous years. C. Smart Start Grant Application Renewals (4 projects) The Health Department has traditionally applied for and received four grants from the Orange County Partnership for Young Children (OCPYC). In 2008 all of our grant projects will require a full application to the OCPYC. Budget instructions indicated we could request up to 85 – 90% of total project cost. The projects are as follows: Help 4 Kids: Funding for a Nurse Coordinator, plus operating expenses, to serve as a central portal of intake for information and referral regarding services for children 0-5 at risk for or with actual developmental delays. Current funding = $56,760; Requested funding 08-09 = $71,054, 09-10 = $73,510, 10-11 = $76,027. Families in Focus: Funding for a nurse, plus operating expenses, to provide intensive home visiting services to high-risk families with children age 0-3 to prevent child abuse/neglect. Current funding = $62,372. Requested funding 08-09 = $74,140, 09-10 = $76,044, 10-11 = $78,383. Child Care Health Consultant: Funding for a nurse certified as a Child Health Care Consultant to provide health, safety, and basic nutrition and physical activity services to childcare centers (3 star) and family childcare homes. Current funding = $57,596; Requested funding 08-09 = $72,787, 09-10 = $75,284,10-11 = $73,596. Dental Preschool Screening: Provides screening and referral services for children with identified dental health needs in child care centers and family care homes. Targets 3- star centers and homes first and then does others interested in the screening. D. 2008 Community Health Assessment Approval The final 2007 Orange County Health Assessment that was submitted to the state was provided to Board members in CD format for information and approval. E. Acceptance of Kate B Reynolds Grant Funds (Healthy Carolinians) This is a grant award of $30,000 over three years. Funding from Kate B. Reynolds will assist Orange County with the promotion and marketing of the Healthy Carolinians Partnership, partnership and leadership development efforts, and will provide needed administrative support for the Healthy Carolinians Coordinator and committees. F. Acceptance of Additional Funds The NC Division of Public Health (NCDPH) has provided additional funds for Pandemic Flu preparations to local Health Departments for FY 07-08. The amount awarded to Orange County Health Department is $20,000. If the Health Department is unable to spend the allocated funds by May 31, 2008, it will result in a loss of the unspent funds. Motion to approve the minutes of November 13, 2007 and items on the Consent Agenda was made by Tim Carey, seconded by Moses Carey, and carried without dissent. V. Reports and Discussion with Possible Action A. Election of Officers for 2008 MINUTES ORANGE COUNTY BOARD OF HEALTH December 20, 2007 Board of Health Minutes Transcription completed by Anne Miles Cassell 3 December 20, 2007 The operating procedures of the Board call for the Chair and Vice-Chair to be elected annually. The Nominating Committee presented the slate of Chris Harlan as Chair and Anissa Vines as Vice Chair. There were no additions or changes offered. The elections are required to be by roll call of the members present at the meeting for each position. A simple majority of those present is required for election. An individual voice vote was taken and recorded for each candidate and position. The results were unanimous for Chris Harlan as Chair and Anissa Vines for Vice-Chair with no dissenting votes. B. Position Requests for FY 2008-09 Rosemary Summers reported that the Department requests the Board consider approving the following positions for county consideration, one for the remainder of this fiscal year and the rest for FY 08-09. The position request for immediate consideration is due to the arrival of over 107 refugees from Burma since July 1, 2007 and an additional 200 refugees are expected from Burma, Iraq and other countries in 2008. Wayne Sherman reported that there are several refugee resettlement agencies in the area that are planning to resettle at least another 200 refugees in Orange County through September 2008. All refugees qualify for 8 months of Medicaid when they arrive and must be screened for communicable diseases and TB. An initial physical assessment screening is done by the health department and many refugees have significant healthcare needs due to their past lack of health services. Moses Carey asked how the department was notified of new refugees and what the time limit is for the CD and TB screening. Mr. Sherman replied that the refugee resettlement agencies notify the department and that there is a 30-day time requirement for the initial screening. Dr. Carey inquired about translation/interpreter needs for the refugee community. Chris Harlan commented that many different languages are used in Burma, not all Karan. 1) Beginning February 1, 2008 and continuing thereafter, Office Assistant II for Refugee Health, Health Promotion and Quality Improvement support. ($41,578) Dr. Summers reported that new recurring aid to county funds and recurring funds earned through Medicaid administration match would fund the majority of this position. 2) Full-time dental hygienist position for preschool screening project coordination and for serving prenatal patients and other adults. ($98,945) This position is partially grant funded through Smart Start. 3) Half-time nurse practitioner to provide comprehensive health assessments at SHSC on arriving refugees within 30 days. ($12,248 county cost) 4) Public Health Nurse II to meet increased demands in the communicable disease and refugee health programs ($67,718) 5) Renewed request for permanent funding for Volunteer Coordinator position for the Medical Reserve Corps (need an additional $22,000 in permanent program support). Motion to approve position requests as presented was made by Anissa Vines, seconded by Ernie Dodson, and carried without dissent. Clean Water Management Trust Fund Grant Application Former Board member Alan Rimer has worked with staff in years past and prepared a concept paper proposing a program to monitor and mitigate impacts to protected watershed areas from failing septic tank systems. The program includes educational components, homeowner system management through incentives, and a management program that includes more extensive inspections of existing on-site systems on a prescribed periodic basis. The grant request to be submitted to the North Carolina Clean Water Management Trust Fund (CWMTF) would be for a two-year funding period and includes the addition of 5 staff members. Because the completed applications are due by February 28, 2009, and the application forms were only posted the week of 12/10/07, it leaves little time to prepare the information necessary to meet the deadline. In addition, the Board of Commissioners must approve the pursuit of these grant funds prior to application. Part of the grant funding requires matching funds by the County therefore a commitment to this program by the County will be necessary for success. Moses Carey asked if the matching funds would be required the first two years by the County. Dr. Summers responded that in-kind may be a sufficient match. The Board of Health was asked to consider the concept of this program prior to further pursuit of funding, which will likely be in a future CWMTF cycle. Tim Carey voiced his support of the program. Moses Carey asked how replacement of failed septic systems would be funded and Anissa Vines wanted to know how many were expected to fail. Mr. Konsler responded that the loan repayment fund would be developed as part of this proposal. The expected failure rate depends on a number of factors but usually is thought to be about 10 – 15%. Motion to approve the pursuit of grant funding by CWMTF and forward to the Board of Commissioners for action was made by Moses Carey, Jr., seconded by Anissa Vines, and carried without dissent. D. Informational Items 1) Influenza Update – Wayne Sherman Wayne Sherman reported that Orange County has had one case of A/H3 flu reported as of December. The Department has administered 1,850 vaccines. There is still vaccine available at the clinics and will continue to be offered to the community. Our “surplus” will be offered to the sheriff for the jail population. 2) Accreditation Update – Carla Julian Carla reported that we have 90 days from January 2 for submission of our documentation. Our site visit is scheduled for the last week of April. 3) Clinical and Community Program Committee – Jim Stefanadis MINUTES ORANGE COUNTY BOARD OF HEALTH December 20, 2007 Board of Health Minutes Transcription completed by Anne Miles Cassell 5 December 20, 2007 Jim Stefanadis reported that the outbreak of 8 additional TB cases since July in the County was the most notable topic. The minutes from the CCP Committee were provided for Board members. E. Tracking Form Review The Tracking Form has no changes. This form provides Board members a timetable of upcoming agenda items.
