THE ORANGE COUNTY BOARD OF HEALTH MET ON February 22, 2012, at the Whitted Human Services Center, Hillsborough, NC. Approval of February 22, 2012 Agenda Motion to approve the Agenda of February 22, 2012 was made by Steve Yuhasz, seconded by Matthew Kelm, and carried without dissent. II. Public Comment for Items NOT on Printed Agenda: none III. Action Items (Consent) A. Minutes approval of January 18, 2012 meeting The Board is required to review and approve minutes of all official meetings of the Board. The latest minutes are from the meeting on January 18, 2012 Motion to approve the consent agenda items was made by Corey Davis, seconded by Steve Yuhasz, and carried without dissent. IV. Customer Satisfaction Survey The Department collects feedback from customers at different points through out the year. As the information is analyzed, strategies may be devised that will improve services in response to the data. As required by accreditation, annually, the managers present a summary of the analysis and any changes made to improve services. Management staff presented these results at the Board meeting. Reports and Discussion with Possible Action A. FY 12-13 Budget The proposed budget for the Health Department for FY 2012-2013 is essentially a “flat” budget, with minimal increases. When the vehicle capital request, proposed new 0.5 FTE in Environmental Health, and involuntary increases are removed, there is actually a decrease in county appropriations. The proposed operating general fund budget is $7,327,880, with an additional $122,718 allocated for two grants. The grand total is $7,450,598. The fiscal outlook from the state is still unclear, with guidance to budget similar amounts as appropriate in the current fiscal year. The exception is a definitive loss of Health Promotion funds as well as the anticipated loss of state tobacco money. Staff will continue to closely monitor the state budget discussions in the legislature. In the attached FY 2012-13 division budgets, multi-program divisions have allocated expenses and revenues by program. This information will better capture the cost of service delivery while examining its correlation to better health outcomes for our clients. Commissioner Yuhasz asked if we could provide a graph that shows the percentage of county and non-county appropriations. Staff will put together another graph and send out electronically to board members. A motion to approve the general fund budget in the amount of $7,327,880 for 2012-2013 as presented and the grant fund budget in the amount of $122,718 for 2012-2013 as presented and forward to the Board of County Commissioners for action was made by Susan Elmore, seconded by Anissa Vines, and carried without dissent. B. FY 2012-13 Fee Schedule The new Medicaid Cost Settlement methodology required that the Health department examine its existing fees. In doing so, staff used the following methodology: 1. Increased fees to the regional cost average rather than our actual cost if it was significantly higher 2. Decreased fees if our actual cost was less than our current fee 3. Confirmed that all fees are equal to or exceed the Medicaid reimbursement rate This resulted in an increase of 42 fees and decrease of 14 fees for the medical clinic and an increase in 43 fees for the dental clinic. Additional fee adjustments: • Recommend an increase in the minimum fee for medical nutrition therapy from $15 to $20. This will align the minimum fee more closely with Personal Health Services primary care minimum fee of $20. • Establish new fees as a result of a mandatory Medicaid changes in global obstetrician billing packages effective October 1, 2011 instead of billing per visit. • Changed minimum fee for dental service from $30 per visit to $30 per procedure. This brings our fee structure similar to other dental clinics like Alamance County. C. Strategic Plan The Board of Health Members reviewed the proposed priority summaries Access to Care According to the 2011 Community Health Assessment, residents of Orange County have less than ideal access to health care services. Issues such as lack of awareness of resources, low health literacy, insufficient cultural competency, and sporadic transportation availability provide barriers to accessing the right medical service at the right time. The Board of Health will work to address this issue through 4 focus areas: 1) Serving as a catalyst for health care partnerships in the county; 2) Advocating for and pursuing policies/legislation at the local, state and national level that improve access to health care; 3) Fostering a culture of innovation at the Health Department so that new ways of addressing this issue can be tested and developed and 4) Actively communicating about effective interventions and advocating for their funding. Child and Family Obesity Obesity remains a top health concern for Orange County. Primary prevention of obesity by working with children and their families remains the most effective intervention. The Board of Health will work to address this issue through 3 focus areas: 1) Serving as a catalyst for obesity prevention partnerships in the county; 2) Identifying family specific, evidence-based nutrition and physical activity interventions that can be implemented throughout the county and 3) Advocating for and pursuing policies/legislation aimed at reducing obesity through environmental and regulatory changes. Substance Abuse/Mental Health Substance abuse and mental health issues permeate everything local public health does. From exposure to environmental tobacco smoke causing an increase in cardiovascular disease to the challenges associated with ensuring medication compliance in a mentally ill patient with a communicable disease, both have far-reaching health and economic impacts and addressing these problems will take a fully engaged and active presence by all organizations serving these clients for whatever reasons. The Board of Health will work to address this issue through 4 focus areas: 1) Advocating for the elimination of smoking in all public places in Orange County; 2) Advocating for and pursuing policies/legislation aimed at improving substance abuse and mental health services; MINUTES ORANGE COUNTY BOARD OF HEALTH February 22, 2012 Board of Health Minutes Transcription completed by Lisa Smith February 22, 2012 Page 4 3) Exploring, with our partners, the challenges and potential solutions of integrating mental health services into the primary medical care office and 4) Exploring, with our partners, how to better coordinate care for substance-addicted and mentally ill clients. Board members volunteered to serve on the 3 teams that are working on developing the Board of Health Strategic Plan. Each team will develop the goals for their board of health priority. Access to Care Committee: Matthew Kelm and Tony Whitaker Child and Family Obesity Committee: Carol Haggerty and Mike Carstens Substance Abuse/Mental Health Committee: Corey Davis and Anissa Vines There will be 3 meetings set up for each group which will consist of board members and staff beginning the end of March and ending in mid May. D. Outside Agency Funding A review along with a completed scoring sheet needs to be available to the Health Director for presentation to the County Manager when the departmental budget is presented for his review. Generally, this happens sometime mid to late March. The Health Department received nine applications. We need three groups with 3-4 BOH members to review and score three applications they same way as last year. The applications, financial information, score cards, and instructions will be sent to each of you and will need to be sent back to Lisa by March 16. E. Informational Items Informational items are provided to the Board to cover topics that Board members should be aware of or for general interest.
