ORANGE COUNTY
AMENDMENT TO SERVICES AGREEMENT
NORTH CAROLINA
THIS AMENDMENT, made and entered into this the first day of January, 2017, by and between the County of
Orange, a body politic and corporate of the State of North Carolina, ("County"), and Empowerment, Inc.
("Provider");
WITNESSETH:
WHEREAS, the County and Provider entered into a Services Agreement dated, July 1, 2016, to provide services to
be rendered by Provider to County's Health Department("Original Agreement"); and
WHEREAS, the County and Provider desire to amend the Original Agreement, while keeping in effect all terms
and conditions of the Original Agreement not inconsistent with the terms and conditions set forth below.
NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, the parties
agree to amend the Original Agreement as follows:
1. Amend Section 3.a as follows: The County agrees to appropriate for the provision of services described
in Exhibit "A", Scope of Services and more particularly described in the Program Budget Categories,
the maximum sum of Eighty Nine Thousand Eight Hundred Forty One dollars (S89,841).
2. Amend Exhibit A, Scope of Services, to add: (See Attached Amendment to Exhibit A, Scope of
Services)
Except for the changes made to Section 3.a. Funding and Exhibit A, Scope of Services, the Original Agreement
shall remain in full force and effect to the extent it is not inconsistent with this Amendment. In the event that there
is a conflict between the Original Agreement and this Amendment, this Amendment shall control.
IN WITNESS WHEREOF, Orange County and the Consultant have signed this Amendment, effective this
the 1st day of January, 2017.

Family Success Alliance will provide additional funding to allow zone navigators to work an
additional 8 hours per week from January 2017-June 2017 for a total of 28 hours per week (.7
FTE). With the additional hours, navigators will:
• Recruit between 15-18 families into navigator program.
• Assist with recruitment for K Readiness (with minimal assistance to those families re
expectations for Kindergarten —e.g., immunizations)
• As applicable, meet with cohort one families on a monthly (as opposed to bimonthly)
basis if that better meets family needs.
Personnel: 3 zone navigators
Additional Cost: $14,551
Budget Categories
1. Salaries: $11,232 (3 staff paid $18 for up to 8 additional hours per week for a six
month time period)
2. Taxes: $1,002 (FICA/Social Security- 0.062, Medicare - 0.0145, NC
Unemployment- 0.01272)
3. Overhead: $1835 (15% of salary plus taxes)
4. Mileage: $482 (@.535/mile for up to 50 additional miles per month per
navigator)
Additional Provider Deliverables:
1. Ensure that each navigator successfully recruits 15-18 cohort 2 families.
2. Review with navigators, activities related to K Readiness recruitment and share
any reported challenges with FSA Program Manager.
