Passmore, ext. 2009 TELEPHONE NUMBER- Hillsborough - 732-8181 Chapel Hill - 968-4501 Mebane - 227-2031 Durham - 688-7331 PURPOSE: To request approval to add Piedmont HIV Healthcare Consortium to the participating agencies in the Coordinated Area Transportation (C.A.T. ) system and Chair to sign Agreement. BACKGROUND: The C.A.T. system, administered by the Orange County Department on Aging, provides drivers and vehicles to transport program clients. The C.A.T. agreements clarify the benefits and responsibilities of the parties involved. Piedmont HIV Healthcare Consortium first inquired about obtaining services from C.A.T. in 1992 . The Participation Agreement would formalize this coordination. The cost-per- trip would be consistent with other contract agencies ($12.25 per person each way) . RECOMMENDATION(S) : The Manager recommends approval of inclusion of Piedmont HIV Healthcare Consortium in the C.A.T. system and Chair to sign Participation Agreement. 2 ORANGE COUNTY C.A.T. PROGRAM AGENCY PARTICIPATION AGREEMENT This agreement entered into this 15th day of November, 1994 by and between the County of Orange on behalf of the Department on Aging, hereinafter referred to as the County and the Piedmont HIV Healthcare Consortium, hereinafter referred to as the Piedmont Consortium. The purpose of this agreement is to outline the conditions under which the County who administers the C.A.T. (Coordinated Area Transportation) Program and the Piedmont Consortium will cooperatively provide human service transportation to Orange County's elderly, infirmed, and disadvantaged as a participating member of the C.A.T. Program. SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY 1. To assign professional staff within the Department on Aging to administer and coordinate the C.A.T. Program. 2 . To monitor CAT Two-Way Radios installed in participating agency vehicles. 3. To arrange bulk gas purchases for agency CAT vehicles. 4 . To provide short term backup loan vehicles for Agency CAT vehicles temporarily out of service. SPECIFIC RESPONSIBILITIES OF Piedmont Consortium 1. To operate and schedule the use of participating C.A.T. agency vehicles(s) on behalf of human service clients and approved groups, according to the Orange County Transportation Board policy and procedures. 2 . To allow C.A.T. Program agencies access (time/ridesharing) to assigned vehicle(s) where appropriate and not scheduled for use at a rate established by the County Commissioners. 3. To complete all required vehicle operation reports and submit to the Orange County C.A.T. Supervisor in the Department on Aging. PROGRAM DRIVER ASSIGNMENT AGREEMENT This agreement entered into this 15th day of November, 1994 by and between the County of Orange on behalf of the Coordinated Area Transportation Program (C.A.T. ) , hereinafter referred to as the County, and Orange County Middle School Afterschool Program, hereinafter referred to as Orange County Schools. The purpose of this Agreement is to outline the conditions under which the County and Orange County Schools will cooperatively provide human service transportation through the C.A.T. system. SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY 1. To provide a certified CDL driver for the following route( s) as described: Five routes transporting Afterschool Program participants home from A.L. Stanback School each Monday through Thursday for the balance of the school year. 2 . To provide a back-up driver in case the regular driver is not available. 3 . To employ a C.A.T. Transportation Supervisor to handle any problems and to assure there is a driver available. SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY SCHOOLS 1. To reimburse Orange County at the rate of $10 .00 per hour for actual driver time plus $20. 00 per hour for 2 .5 hours of administrative personnel time within 30 days after billing. 2 . In the event the agency requires specialized training beyond driver's current level, it shall be their responsibility to: a) indicate the type & level of training; b) provide the prescribed training without cost to the County. 3 . To provide the Transportation Supervisor written notification of any transportation problems or unsatisfactory drivers' performance in order to take corrective action. The term of this agreement shall be from November 15, 1994 through June 30, 1995 unless sooner terminated by either party giving 30 days prior written notice to the other party of intention to terminate. The laws of North Carolina shall govern the validity and interpretation of the provisions, terms and conditions of this Agreement.
