At a meeting of the Board of Social Services = Nay 28, 1981, the Board.
passed a motion in support of the enclosed request for 'two large rooms for a
day care center in the Cedar Grove Multipurpoee Center. In addtion, the Board
supported Option I, the use of two rooms in the Headstart wing, in light of
the large differences in cost and the drastic outs fectng other social services
programs. Descriptions of the options presented follow.
Option I -- Two rooms in the Headstart wing
Presently Headstart has available six large classrooms and two smell office size
rooms on the North wing. Three of the large rooms are used daily as classrooms,
one room is used as a gross motor room and has equipment set up in it for this
activity, one large room is used as a parent resource room and, a room .here child
ran can be takenfor testing or therapy, and the other large room has a dental
chair in it and is used for dental hygiene and, also for testing or therapy. The
smaller two rooms are used very infrequently.
Headstart has agreed to give up one room on the North wing to the day care center.
This room would be suitable for the infants and toddlers on one aide and the two's
and three's on the other. However there is no space for either the children who
will remain after Headstart or for expansion of a three—four year old, class the
folleving year. Headstart is not willing to give up another large room
but would be willing to share the gross motor room with the after-Headstart day
care children. Renovations necessary for the daycare center to function on this
wing would include installation of hot water, painting and pipes insulated. It
is estimated to cost $1,200.00 for parts and labor and could, in all probability
• be completed by county crews. Advantages to this option are mininal costs,
limited readvationa, rooms already assigned for young children with appropriate

plumbing fixtures. Disadvantages includes disruption and reorganization of Head-
start space and problems involved actually sharing afface. In addition, if the
canter only had one room and shared space with Headstart for use after Headstart
was not in session, then there would be no possibility of two and, three year 'old
class later becoming a three and four year old class. And next year at this time
the center and Headstart could possibly be struggling over space again.
Option II'—Two rooms on the Southern wing
There are two rooms presently vacant next to the bathrooms on the southern wing.
Neither has plumbing for sinks and commodes. In order for the center to locate
in those two rooms the following renovations would, need to be made:
installation of a septic tank, installation of plumbing and, fixtures for two
sinks and commodes including hot and, cold, water connections and, connection to
septic tank, new flooring, fire detection system, painting, and pipes insulated.
This ia rongIlly estimated to cost between $7,000 - $8,000. In addition much of
this work would have to be contracted out and not done by county crews. This

would make meeting an August deadline difficult. Advantages to this option is
little or no disruption of present activities and. programs presently in the
building and the separated space for the day care center. Disadvantages include
both the sizeable cost and length of time needed for renovation.

Request for Space for a Day Care Center in the
Cedar Grove Multipurpose Center
The Cedar Grove community and the entire northern end, of the county has wanted
and needed. a day care center for many years. Headstart does not meet the needs
of wOrrl'ing parents because of its limited hours of operation and it's strict
eligibility requirements. In addition, there is no care for children under three
years of age.
For the last several months a group of cormunity people have been meeting to co-
oreinate the activities necessary to make the day care center a reality. Commit
tees have been formed to work on outreach and earollnPnt, fundraising, and the
development of goals, policies, end. procedures. The center would livrP to
begin in September with five infants, sin toddlers, ten twos and threes, and
twelve three to five year olds who would participate in Eead.start when it was in
operation and come to the center other times such as Mondays, after 2:00, and in
the summer.
In order for the group to Proceed with its efforts to raise money and, orgPnize
the center, space in the multipurpose builtiAng must be secured. The day care
center will need two larger classrooms to meet its needs. These rooms need to be
in good repair and have sinks and toilets. One room must have hot water because
babies will be cared for in the center. The community has set a goal of $3,775
to fundraise to start the center. This does not include any cost for renovating
the space needed (see enclosed start-um budget). There is little possibility that
the center can raise more than thiS initial $3,775. Therefore, we are making a
request for two rooms in the Cedar Grove NUItipurpose Center for the Cedar Grove
day care center. lie also request, that the county assume the costs of renovating
these rooms to meet the health, fire, and building standards that would allow the
center to open in September.

RE: Preliminary Approval of Health Budget
We needed to submit an unsigned budget request to the State on May 26,
1981. The Board of Health voted to accomplish this as soon as acted upon
by the Commissioners.
The process is that we will not sign the request until approximately
Juno 15 (after OHS has had two weeks to look it over). The June 15 signed
budget wnuld still be subject to final local, State and Federal appropria-
tions.
I assume that by June 1 you will have the computer printout of our
request, therefore I have not included a copy of our thick budget request.
However, attached to this memo is my analysis of the requested bodget.
I assume that on June 1 you will wish to concentrate discussion on the
amounts, as we are to present our programs at your June 9 meeting in
Chapel Hill.

Comment on 5ecend Draft of Sudaet
On May 19, 1981 , the County Manager indicated that the budget should be
recalculated with a proposed 5% increase in employee salary and a decrease
in the personal vehicle mileage reimbursement to 22d per mile. Economy
County vehicles will cost the department 250 per mile.
On May 16, 1981, the Director discussed Federal/State Funding guidance
with various persons in O.H.S. The product of those meetings was an under-
standing as to which Federal/State Funds were Federal and which were State..
in the MCH program, the mix of Federal/State is variable per department and
non-Formula. As there is no present reason to predict a decrease in the
State funds (which during the past 35 years have been a. small portion of
local health) the immediate Fund cut attention was focused an the Federal
level. The Family Planning Title X(564,337) should not be cut until Fiscal
'32-'33,as this coming year is funded from the '20-'31 Federal appropriation.
Probably, there are those in the Federal administration who will try to
apply the block grant and 25% cut ex post Facto, due to their philosophical
opposition to Family Planning. However, the OHS guidance is to assume this
will not happen and to budget at a continuation level. Lastly, regards the
eamaining portion of Funds identified as Federal, the Director would advise
cutting those 53,170. In summary, assuming the State Funds will not be
cut and that Family planning will not be cut until FY '82-'93, the total
State/Federal cut is 55,170.
Most important to the budget is a special aid-to-counties State grant
of 536,000. This money was originally obtained From the State Legislature
two years ago due in significant part to the Local Health Oirecter's
Asacciation and with help of others such as the Orange County Ccmmissioners.
Next year, this money is included in a general State-to-County revenue
shoeing bill which locks very good For passage. This budget assumes the
bill will pass and the Orange County Commissioners All continue the
536,000 per year to the health department, even though the money would lose
specific identity for local health in the present bill_
At the beginning of this fiscal year the department reduced its
eteff by one nurse and one sanitarian. Mid-year, a vacant health educator
peeition was converted to a clerk in reaction tc an oceerations crisis.
This budget, additionally, dces not replace a Frozen clerk and a second
health educator position. However, the total drop appears as two less,
since two Accounts Clerk III are transferred in Frcm another county
deeerteent due to the dictates of logic; they wctked fell time at the
health department. This transfer is seen as an increase in the health
budget offset by a decrease in another county departs budget.
The department will be hard pressed by increased demand next year, as
general. economic problems result in more people expecting service. The
department already has had to cap clinic growth and reorient nursing activities,
The buogec does not include staff expansion necessary to increase the eent=1
program to more than 1/4 time at each effice . Lastly, the general community
based health education effort of the department will only be able to coerste
at approximately 1/3 of its level of six months ago.

III.
IMPACT OF BUDGET REQUEST ON SERVICES
Dental Health
The failure to add a dental assistant and a clerk will result in split
time clinical operation between Carr Mill and Hillsborough. The single
site (Carr Mill) operation at this time is reduced by a factor of 20 ,
However , this budget will restore the case load to normal. It is
unfortunate to not be able to expand services to more needy residents this
fall than presently served . The Dental School representatives state
that they could furnish dentist attendings and residents to run both clinic
three plus days per week, if the County would furnish the necessary
assistance. The cost to the School for expansion would range from
527 ,000 to 831 ,000 , depending on salary levels . The cost to the County
to expand service would be approximately $29 ,000 for staff and ooeretory
supplies . A conservative estimate is that the increased operational time
would generate approximately 810 ,000 in fees at the present rate structure;
therefore, the net county cost would be approximately $19 ,000. A
conservative estimate is that the increased time would increase the annual
patient clinical visits to a county total of 6 ,000. With Sufficient Lead
time , the Dental School could possibly staff each clinic full time at
no required further significant County outlay. With that significant
patient increase the net County cost to make the assistance staff input
at this time would drop to a low Level .
Environmental Health
Although this effort is staffed one person Less than it was at this
time lest year , the program should be able to continue without interruptio
or service reduction. Two of several factors which have made this possible
are that the new building starts have dropped off and the three used car
radios we purchased have increased our efficiency. This program is
recognized for its quality and this budget would be a major factor in
maintaining that quality.
Health Education
Community health education by a speciality staff of professional health
educators will be reduced by 2/3 of its Level of six months ago by this
budget . This mission of the health department will resultantly be
dramatically refocused from a speci =lif-y effort to an operational mode for
li_ne staff, The remaining health educator will have to focus on stimulatin4
=ne training our direct service staff to accomplish their duties in a.
manner conducive to community health education. Secondly , to work with
the ABC funded (to the school systems ) health kucator/counselor and other
epprooriete school systems staff to maximize their efforts . The school
oyetems will, be asked to emphasize health education . Lastly , health
educator direct patient involvement will, be necessarily limited to fan :
plannieg , teens and hieher risk patients .

Personal Health Services
Assuming that the County grants a sufficient amount of the increase
equested , it is planned to refill the Public Health Nurse I.T. - School
,iealth position on August 1 for the 11 remaining months of this fiscal
year (cost $16 , 108) : Also , assuming the one-person-short clerical staff
is successfully reorganized , trained and increased in efficiency:
Lastly, assuming the County allows departmental use of the computer center
then, the clinical and other personal health programs will not suffer
any program terminations .
However, the general economic situation has resulted in a dramatic
increase in public demand for service. The department attempted to -
accomodate the public so far this fiscal year despite the fact that we
have one less nurse than at this time last year. All clinics will shortly
have an appointment system which will limit the clinics to their present
size or less . It is a safe prediction that this will result in longer
waiting lists to become a new patient and less immediate appointments
to existing patients , Secondly, the new patients registered will become
increasingly restricted to priority population subgroups such as teens ,
high risks , etc. . The human service medical care cutbacks of the present
Administration will result in many persons not being able to access care
through other sources , The preventive services of this department
are focused on women and children who will bear the brunt of the general
Federal cutbacks. Most of those not already a department patient will
also not be able to get help from the Health department. Lastly, the
epartment will not be able to focus on others who will also nee4essistance,
A priority- of most health plans is in-home-services to reduce the '
_taggering cost of institutionalization. The health department will reduce
its in-home-service by about 50%-70% in order to handle other duties with
its smaller nursing staff. Provision of in-home-service is considerably
more expensive than congregate sites (i,e. , clinics , schools , etc. ) .
Secondly , recent County decisions indicate that we will be budgeted
inadequately for the necessary travel to provide in-home-service. Unfortunately,
the Administration's plans will result in many other agencies also decreasing
in-home-care . The long range result of this is predictable to be greatly
increased institutional care and cost.
The level of care that we will be able to continue to provide with this
budget is grossly inadequate by any objective measure . Our school health
crogram will only provide six hours per week per school of nursing time ;
y or Lass of low income/high risk pregnancies will be clinically aided
by the department ; 1/3 of the children we should provide well child clinic
service tc will be so served ; 1/10 of the sexually transmitted disease
oases we should attend to will be helped ; and , outreach activities to
reach new persons have practically stopped , since we can not provide
service to them anyway.
In illustration Of the foregoing points , with decrease of one nursing
noeition in FY 80-81 , the following increase of activities have occurred :
1 , Setwee,r, FY 79-80 and FY 80-to date , an increase of approximately 75!.,
in Tenereal Disease treatment within Health Department Clinical Se' v_c
From FY 79-80 to FY 80-to date, an increase of 17% in Child Health
Clinical Services in Hillsborough; an increase of 12% in number of
children served.

RE: Appeal of Frozen Position
Early last week I was notified that a recent clerk hire had decided to
resign due to a variety of reasons. This is our medical records clerk/lead
clerk position in Hillsborough. This position is extremely key to our
clinical operations and must be filled. Whereas we have not budgeted some
frozen positions for next year, this one,we have.
I would appreciate your action on June 1, as I will have already lost
two weeks from time of resignation towards filling the job. Thank you for
your attention to this matter.

2 AN ACT TO AMEND VARIOUS SECTIONS OF THE GENERAL STATUTES TO
3 ESTABLISH EXEMPTION FROd THE STATE PERSONNEL SYSTEM OP CERTAIN
4 LOCAL GOVERNMENT EMPLOYEES.
5 The General Assembly of North Carolina enacts:
6- Section 1. G.S. 126-5(a) is amended to become G.S.
7 126-5 (a) (1) .
8 Sec. 2. G.S. 126-5 (a) is further amended by adding a
9 new subsection to read:
10 " (2) The executive heads of local government agencies and the
11 positions reporting directly to them which are primarily
12 responsible for both substantive program policy determination and
13 its public advocacy, confidential assistants to these positions,
14 and attorneys serving as legal counsel may be exempted from
15 coverage under provisions of this Chapter, except_frrZIcIe 6,
16 when so requested jointly by aboards,--E- Llity commissioners and
17 the program board concerned, such request to be made in
18 accordance with rules established by the State Personnel

12 "according to the merit system rules of the State Personnel
13 Board" and inserting "in accordance with rules of the State
14 Personnel Commission when applicable. "

We have enjoyed the assistance of a number of graduate students
in the Master of Public Administration program at UNC, who have
worked at the direction of the Human Services Advisory Commission
4unding Committee to make a study of funds coming into Grange
County. They have analyzed the Department of Social Services
and JCCCA.
Their report has been completed, and has been shared with Kathy
Battern, County rinance Director, who was most enthusiastic, and
felt that their work had been greatly helpful. The FAO Fundin'
Committee feels that they have done an excellent job, which
will be invaluable in setting up a format for analyzing the
fundimz,, qf all human service agencies in the enunt . _
/7 in the process of their work for the SAC, these students have
incurred expenses for transportation, copying, postage, etc.,
and I would like to request a reimbursement of 400 to help
in the defraying of their costs p and request the Commissioners
to appropriate this amount, payable to Margot Gold, Department

The persons whose names appear on the attached page performed at
the request of the Human Services Advisory Commission. The study deals
with funding of human services programs in Orange County.

Altogether, over 100 man hours were consumed in developing the materials for the

RE: Funding of the Industrial Authority/Metro Atlas
DATE: May 26, 1981
The Research Triangle Marketing Committee has agreed to participate in the
development of a Metro Atlas, a 45 to 50 page publication to help recruit
new business and industry to the area. The project will cost $I0,008 and
the Committee is asking each of the participating counties to contribute
on a prorated basis, based upon the new census figures. Orange County's
share will total $897.00 . It is my understanding from the latest Minutes
of the Orange County Industrial Authority, that the Authority has agreed that
this is a good investment.
It is my recollection that the Board appropriated approximately $1,000 for the
use of the Industrial Authority, at the request of Mr. Shannon Julian, then
Chairman of the Authority. However, the Finance Department has not been able
to locate this appropriation in the budget. If the Board wishes to endorse
this expenditure for the Metro Atlas, I suggest that the money be appropriated
by the Board from Contingency into the Industrial Authority account so that
the check might be issued without further delay.

also known as Lindley Drive, North Carolina State Road 0207, also known as
Welcome Drive in the Rangewood Subdivision located in Chapel Hill Township,
Orange County, North Carolina. The amount to be financed by Orange County is
estimated to be $22,200 including costs and attorney's fees. Approximately
55% of the cost of paying the above roads will be borne by the Department of
Transportation of the State of North Carolina_
2. The basis for assessment for the local share of the cost of this
project shall be the cost to Orange County per linear foot of street frontage,
including County administrative expenses and attorney's fees.

the basis set out in Number 2 above which local share represents approximately
45 percent of the total project cost.

Within the thirty (30) days after Notice of Confirmation of
the Assessment Role is published, each owner of the property may pay his

with interest, due sixty (60) days after the date that the Assessment Role is
confirmed, and the remaining installment, with interest, due on the same date
in each successive year until the assessment is paid in full.
IT IS FURTHER RESOLVED AND ORDERED that a public hearing on all
matters covered by this preliminary assessment resolution will be held on
19 at o'clock P'M', during the regular meeting of

Board passed on May 14th the recommendation that all
Homemaker-Home Health Aides Clients (full pay or those
receiving a free senior worker) be charged mileage for
the aides at the prevailing county rate . (Attachment)

l Title III Homemaker Personnel - This is excess budgeted
funds for reclassification of the Homemaker Cuurdinatur
position but will not take place until July 1 , 1981 .
2 Title III Homemaker Travel - These increased funds will
be used to pay for travel incurred by aides to clients '
homes through April 21 , 1981 . (The overexpenditures of
the Homemaker-Home Health Aide Travel will be charged to
the Title III Homemaker Travel Account. )
3 Title III - I & R - Office Supplies - Paper supplies
purchases were lower due to the County Central Services
providing xeroxing services (including paper) in its
budget-
4 Title III - I & R - Postage - Cost for bulk rate permit
mailing as a county' department rose from 3'5¢ to l0'4t
_ per piece . Thus , mailing costs of 3000 newsletters in-
creased more than $207-OO'
5 Title III - Transportation - Travel - Since most clients
have been transported on DoA or leased vans , excess funds
anticipated to pay staff or voluoteers� for client travel
have accumvlated.
§ Title III - I & R - Travel - Several unanticipated Triangle
J COG workshops have been held over the past six months ;
also, funds will be needed for staff to attend the Division
of Aging Annual Conference scheduled in June rather than

due to Bernice' Hupkivs , Program Director, voluntarily re-
ducing her work hours over the past six months . Mrs .
Hopkins will be officially retired as of June 30, 1981 '

thus reducing social security funds .
9 Capital - Van Replacement - The $2000 budgeted for' a van

that the funds be used toward Capital renovations to the
proposed DoA office space in the Chapel Hill Multipurpose

to the Chapel Hill Multipurpose Center - Mr. McKerrow moved
recommendation to relocate the Carr Mill office to the Chapel Hill Multi-
. purpose Center effective July 1 , or earlier if required, seconded by

Gloria Williams . Ms. Williams stated in view of past discussions
of the group studying the move of the office to the Multipurpose Center,
the group was moving toward the establishment of a Southern Orange
Senior Center at the Multipurpose center. Ms. Williams added she would
like this concept included in this motion. Mr. McKerrow stated that
he would not want this board at this time to locate the Southern Orange
.
Senior Center at the Multipurpose oenter, Ms Williams amended the

motion on the floor adding - and attempt to use the facilities to evaluate
the possibility of a Southern Orange Senior Center on a trial basis .
Hr. McKerrow accepted the ammendment, Carried. Motion Carried.
Item 4, Review and Approve the revised draft of Operational ByLaws
for the Orange County Senior Citizens Board - Postponed issue
to next meeting.

Item 5. Review and Comment on DoA Budget narrative for the New Fiscal
Year July 1 , 1981 - June 30, 1082 - Mr. Passmore summarized
the budget information distributed to Board members present. Mr. Passmore
commented that basically all programs were remaining the same as last
year. The RSVP program was attached, but not included in this budget
issue since the matter of RSVP Will be a decision of the County Commissiooer
Mr. Passmore called for the pleasure of the Board in respect to submitting
the budget as prepared and possible eliminations if required. Mr. Kenyon
moved to submit the budget as is to the County Manager, Seconded by
Gloria Williams. Carried.
Item 6. Focal Point Designation of Facilities in Orange County -
Issue postponed to next meeting.
Item 7. Directors Report 7 Mr. Passmore briefly summarized items as
listed in agendn. He stated that the Board would have to make a decision
on the the transfer of funds to renovate part of the DoA office in the
Multipurpose Center and cover the deficit in the Homemaker-Home Health
Aide Travel at its next meeting.

.This is to inform you that Karl Markuson has resigned, effective
March 26, 1981, as the Chapel Hill-Carrboro Schools appointment to the Orange
County'Energy Commission.
Please inform the Board of Commissioners of Mr. Markuson's resignation
so that the process of appointing his replacement can begin.
Mr. Markuson has recommended as his replacement Mr. Richard Drake,
Director of Vocational Education for the Chapel Hill-Carrhoro City Schools'
I have -requested that Mr. Drake submit to me a copy of his resume which I
will forward to you upon receipt.

Proposed Position Statement Regarding
State Funding to the County for Human Services
Statement of Concern
1. Federal budget reductions have severely reduced Orange
County's ability to provide human services.
2. State government appears to be delaying budget decisions
until the final federal budget is passed, creating
additional uncertainty about local programs and services.
3. Federal and state funding reductions could force counties
out of compliance with mandatory regulations, threatening
to shift the burden for compliance to the local
property tax payer.
4. Regulatory reform, which could provide more flexibility
for local government, is still being discussed and offers
little assistance for the near future.
5. It appears likely that federal funds will, be attached to
block grants provided to the state. But how the state
will provide funds to the counties remains undecided.
6. Maintenance of Effort clauses, such as that put on In-
Home services, restricts the County's ability to respond
responsibly to budget cuts in other areas.
7. Although the Immediate impact of budget cuts can be pre-
dicted in terms of service hours, staff and dollars,
there are likely to be unforeseen consequences to the
- human services system.
As a result, Orange County finds itself in a position of facing
serious reductions in services while having little flexibility
to respond creatively and to reshape its programs to best meet
local needs.
Policy Statement
Orange County takes pride in its history of providing human
services to its citizens. Despite the reduction in funds, the
County can continue to provide effective services if it can have
flexibility in determining local need and allocating funds. To
this end the Orange County Board of Commissioners support the
following policies:
1. Provision of block grants for human services by the state
to the County. Regulations on the use of these grants
should be no more restrictive than federal law and state
statute require.
2. Regulatory reform to allow. the County to distribute blImar
service funds in the most flexible manner. Thia_fenaudes ---
the removal of unrealistic formulas, arbitrary rules and
Maintenance of Effort clauses on funds provided to the
County by the state, with the exception of income main-
tenance programs.

3. Counties that are able and that can demonstrate the
capability to handle local human service decision making
should be allowed to do so.
4. Local government should be involved with the state in
developing guidelines through which block grants can be
distributed in an orderly, effective and least restrictive
manner.
Local Implementation
The Orange County Board of Commissioners will initiate the
following activities in support of these policies:
1. Survey of local human service programs to determine
existing policies and regulations that inhibit or restrict
local decision making with regard to human service
programs.
2. Creation of a legislative action committee to lobby for
Orange County's policies with state government and the
North Carolina Legislature. The committee's goals will
include:
a. Contacts with state officials and legislators to
explain Orange County's position and gain support.
b. Active participation with state government groups
that are presently exploring alternatives for the
allocation of federal and state human service funds . .

c. Development of an information package outlining
the County's position and describing its capacity
to accept increased responbibility for human services
decision making.

d. Request assistance from the North Carolina Association
of County Commissioners to help the committee
develop strategies for its lobbying efforts. ' --
3. Development of a preliminary action plan by the Orange
County Human Services Advisory Commission outlining
alternative methods by which block grants could be
administered through the Orange County government.

C. The Triangle J COG and the Health Systems Agency may each
appoint one member from its own membership,
D. Council members shall represent the interest of the entire
region and shall, to best of their ability, vote for the best
interest of the region as a whole.
Section 2. The county voting membership of the Council shall consist
of one member from each of the following:
A. A member of the appropriate County EMS Council who may be
either a provider or a consumer.
B. A provider or consumer, whichever is not represented by the
appointment from the County EMS Council,
Note: A provider is a representative of one of the following
groups:
1. Doctors
2. Nurses
3. Hospital Administrators
4, Public and Private Ambulance Providers
5, Rescue Squad Personnel
A consumer is a person who does not provide or
administer EMS,
Section 3. Voting members shall have a term of office for two years
with eligibility for reappointment. Voting members who miss three con-
secutive meetings without prior approval may be replaced by the appoint-
.
ing body.
Section 4. Each member appointing body is requested to appoint an
alternate member for each regular appointment of the same provider,
consumer category. Alternate members shall have full voting privileges
in the absence of the regular member. Associate members as defined
in Section 6, may be designated as alternate members.
Section 5. The Council may recommend a replacement for a member to the
appropriate appointing body when that member considers he can no longer
continue to serve or when the member has missed three consecutive
meetings.
