PURPOSE: To accept an Administration For Community Living and Alzheimer's Disease
Initiative 3 year grant of $897,298 for the grant period of September 2015 through August 2018,
and approval of Budget Amendment#2-C.
BACKGROUND: This award given to the Orange County CARES (Community Awareness,
Respite, Educations and Support) program was one of eleven awarded across the nation and
the only one in North Carolina. The funds will enable the Department on Aging to expand
services and supports for caregivers and persons with Alzheimer's Disease and Related
Dementias (ADRD), including those with Intellectual and Developmental Disabilities (IDD), so
they will enjoy improved health and quality of life while also reducing caregiver burden and
social isolation.
The grant will be used to enhance Orange County's ability to be a fully dementia capable
community and will focus on the following objectives:
• Expand both the type and amount of person-centered support services (in-home
consultation/training, group--oriented support activities, and respite services) to
caregivers caring for persons with moderate to severe dementia.
• Provide training to increase the knowledge and skills of direct care workers serving
persons with ADRD.
• Provide training to Orange County government workers and persons employed in
commercial businesses to increase their knowledge, skills and attitudes about dementia,
thereby improving the caregivers' experience of community interactions.
FINANCIAL IMPACT: The required 25% local match for the grant is included in the FY 2015-16
Department on Aging budget for Year 1, and will be included in future department's budget for

the subsequent years of the grant. Budget Amendment #2-C provides for the establishment of
the following Grant Project Ordinance for the receipt of the grant funds:

SOCIAL JUSTICE IMPACT: The following Social Justice Goals are applicable to this agenda
item:
• GOAL: FOSTER A COMMUNITY CULTURE THAT REJECTS OPPRESSION AND
INEQUITY
The fair treatment and meaningful involvement of all people regardless of race or color;
religious or philosophical beliefs; sex, gender or sexual orientation; national origin or
ethnic background; age; military service; disability; and familial, residential or economic
status.
• GOAL: ENSURE ECONOMIC SELF-SUFFICIENCY
The creation and preservation of infrastructure, policies, programs and funding necessary
for residents to provide shelter, food, clothing and medical care for themselves and their
dependents.
• GOAL: CREATE A SAFE COMMUNITY
The reduction of risks from vehicle/traffic accidents, childhood and senior injuries, gang
activity, substance abuse and domestic violence.
Orange County is creating a community that accepts and supports persons living with dementia
and their caregivers; enabling them to fully participate in community life; live independently for
as long as possible; and educating the community on how to protect persons with dementia
from injury.
RECOMMENDATION(S): The Manager recommends that the Board accept the grant,
authorize the Manager to sign the documents associated with the grant award, and approve
Budget Amendment #2-C which includes the receipt of the $897,298 in grant funds and the
addition of 2.625 full-time equivalent (FTE) time limited grant positions.

6. Grantees are hereby given notice that the 48 CFR section 3.908,implementing section 828,entitled
"Pilot Program for Enhancement of Contractor Whistleblower Protections,"of the National Defense
Authorization Act(NDAA)for Fiscal Year(FY)2013(Pub.L. 112-239,enacted January 2,2013),
applies to this award.The effective date is for all grants and contracts issued on or after July 1,2013,
through January 1,2017.
7. All grantees are expected to recognize any same-sex marriage legally entered into in a U.S.
jurisdiction that recognizes their marriage,including one of the 50 states,the District of Columbia,or
a U.S.territory, or in a foreign country so long as that marriage would also be recognized by a U.S.
jurisdiction.This applies regardless of whether or not the couple resides in a jurisdiction that
recognizes same-sex marriage.However,this does not apply to registered domestic partnerships,civil

SPECIAL CONDITIONS
1. Financial Reporting-Grantees are required to submit quarterly and annual financial reports(SF 425),
which are due 30 days following the reporting period.
Quarterly Reporting-Grantees are required to complete the federal cash transactions portion of the
SF-425,lines 10 a through c within the Payment Management System for the calendar quarters
ending 3/31,6/30,9/30,and 12/31 through the life of the award.
Semi Annual Reporting—This report is cumulative. The first report reflects the initial 6 months
period. The report is due 30 days after the 6 month period. For example, awards issued on 9/1/2015,

the report ends on 6/30/2016 and the report for that period is due on 7/31/2016. For each subsequent
report,the end date and due date should be extended by six months. You must reconcile your cash
accounts with your expenditures for the reporting period and submit a cumulative report each year.A
final report is due 90 days after the expiration date of the project period. This report must be
submitted as a"note"using an authorized GrantSolutions account. All grantees must use the Federal
Financial Report(SF-425)form at:

2. Program Progress Reporting-Program Progress Reports are due semi-annually(within 30 days
following each six month period),effective with the start date of the award. This report must be

3. PPHF Reporting-This award requires the grantee to complete projects or activities which are funded
under the 2015 Prevention and Public Health Fund(PPHF)and to report on use of PPHF funds
provided through this award.Information from these reports will be made available to the public.
A semi-annual report that is 508 compliant is required.This report must identify the sub-awardee,
date of disbursement of funds,the Federal award number,CFDA number 93.761 for 2015 PPHF fund
purposes,and amount of PPHF funds.
The reporting cycles are January 1—June 30 and July 1—December 31. Each report is due no later
than 20 calendar days after the end of each reporting period(i.e.,July 20 and January 20,
respectively). This report must be submitted as a"note"using an authorized GrantSolutions account.
4. Cooperative Agreement-This award is issued as a cooperative agreement,a financial assistance
mechanism in which substantial ACL programmatic involvement is anticipated.This award is subject
to the grantee and collaborative requirements and responsibilities set forth in the Cooperative
Agreement outlined in the agency funding opportunity as announced. Those responsibilities are
hereby incorporated by reference as special terms and conditions of this award.
5. FFATA and FSRS Reporting-The Federal Financial Accountability and Transparency Act(FFATA)
requires data entry at the FFATA Subaward Reporting System located at http://www.FSRS.gov for all
sub-awards and sub-contracts issued for$25,000 or more as well as addressing executive

6. GrantSolutions-ACL discretionary grantees are required to use GrantSolutions(GS)for their end to
end grants management services(tracking and receiving various award actions,submitting financial

registered with GS and have the appropriate role assigned to them by their organization. Please

9. Direct Service Requirement-Grantees must dedicate 30%of the first-year budget,40%of the
second-year budget and 50%of the third-year budget to the provision of direct services.
10.Data Collection-Grantees must collect information on services provided under the authority of
existing and future approved data collections.
11.Program Income-Program income must be applied by committing generated income to the project
and used to further eligible project objectives. Program income earned during the project period must
be reported on the annual federal financial report and may be used only for allowable costs in
accordance with the applicable cost principles and the terms and conditions of this award.
12.Planning Phase-Within 30 days of the date in item 1 of Notice of Grant award,grantees must submit
a proposal for their planning phase of not more than six months.During this planning phase,grantees
are required to participate in consultations with ACL staff and the ADSSP technical assistance team.
Grantees will be able to access no more than 15%of total grant funding for developing an
implementation plan. At the conclusion of the planning phase,the grantee must participate in a
"Planning Phase Exit Conference"and receive the approval of ACL/AoA to progress to the
Implementation Phase.The grantee may not advance to the Implementation Phase or access the
remaining 85%of cooperative agreement funding without a"Planning Phase Exit Conference"and
ACL/AoA staff approval of their implementation plan.
13.Implementation Phase—Throughout the implementation phase of the project,grantees will work C'
toward program goal(s),objectives,anticipated outcomes;participate in technical assistance
opportunities/activities (conference calls,webinars)as coordinated by ACL staff and the program
technical assistance team including attendance and participation in activities related to the program
sponsored annual learning collaborative.
14.Sub award-Funds in the contractual direct cost category for the purpose of issuing sub awards
cannot be expended until a detailed categorical budget justification and sub-awardee name is
submitted to ACL within 10 business days of the execution of an agreement. The internal policies
and procedures for issuing sub-contracts must comply with all pertinent Federal regulations and
policies.These documents must be submitted as a"note"using an authorized GrantSolutions account.
