HomeMy WebLinkAbout2016-137-E Health - Mental Health America of the Triangle - Amendment to Navigator Agreement dated 9-18-2015 DocuSign Envelope ID:7190449A-3B32-4622-B7F4-C359EAAB94BA ORANGE COUNTY AMENDMENT TO SERVICES AGREEMENT NORTH CAROLINA THIS AMENDMENT, made and entered into this the first day of January, 2016, by and between the County of Orange, a body politic and corporate of the State of North Carolina, ("County"), and Mental Health American of the Triangle ("Provider"); WITNESSETH: WHEREAS, the County and Provider entered into a Services Agreement dated, November 19, 2015, to provide services to be rendered by Provider to County's Health Department("Original Agreement"); and WHEREAS, the County and Provider desire to amend the Original Agreement, while keeping in effect all terms and conditions of the Original Agreement not inconsistent with the terms and conditions set forth below. NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, the parties agree to amend the Original Agreement as follows: 1. Amend Section 3.a as follows: The County agrees to appropriate for the provision of services described in Exhibit"A", Scope of Services and more particularly described in the Program Budget Categories, the maximum sum of Forty Three Thousand Eight Hundred Seventeen dollars ($43,617) 2. Amend Exhibit A, Scope of Services, to add: (See Attached Amendment to Exhibit A, Scope of Services) Except for the changes made to Section 3.a. Funding and Exhibit A, Scope of Services, the Original Agreement shall remain in full force and effect to the extent it is not inconsistent with this Amendment. In the event that there is a conflict between the Original Agreement and this Amendment, this Amendment shall control. Goal: Zone Navigators will demonstrate the skills needed to conduct effective home visits with clients. • Provide protocols to prepare for and conduct home visits • Review safety strategies for home/community work with families • Zone Navigators will accompany/shadow FAN staff on home visits/client intakes • Accompany Zone Navigators on home visits/intakes, observe and provide coaching and feedback Goal: Zone Navigators will demonstrate an understanding of how to maintain personal and professional boundaries with families and of the impact of self-awareness and self-disclosure when working with families. • Use case reviews as a framework to explore the principles of boundaries, self- awareness, self-disclosure and self-care during group supervision • Provide opportunities for professional development through in-service training, relevant articles for discussion, and community agency site visits Goal: Zone Navigators will help families build protective factors and increase resiliency in their lives and communities. • Provide training on the framework of Resiliency (Risk vs Protective Factors) • Provide tools for discovering existing strengths and protective factors with families • Identify protective factors needed to support individual and family resilience, and strategies for increasing protective factors
